摘 要:目的观察糖尿病性白内障患者行白内障超声乳化联合人工晶体植入术后黄斑中心凹视网膜厚度(FRT)的变化,探讨黄斑中心凹视网膜厚度变化对视力的影响。方法采用前瞻性对照研究方法,选取患有2型糖尿病的年龄相关性白内障患者30例(30只眼)为观察组(A组),无糖尿病病史的年龄相关性白内障患者32例(32只眼)为对照组(B组),正常无眼疾者30例(30只眼)为正常对照组(c组)。A组及B组均行白内障超声乳化联合折叠式人工晶体植入术。常规检查A组、B组、C组术前及A组、B组术后的裸眼远视力、最佳矫正logMAR远视力、眼压、眼前节及眼底情况,并采用光学相干断层扫描仪(OCT)分别检测A组、B组、C组术前以及A组和B组术后1周、术后1、3个月黄斑中心凹视网膜厚度,并进行统计学分析。结果术前A、B、C三组的最佳矫正log—MAR远视力分别为1.06±0.42、0.92±0.37、-0.3±0.06,术后1周A、B两组的最佳矫正logMAR远视力分别为0.35±0.06、0.31±0.34(P〉0.05),术后1个月分别为0.38±0.19、0.27±0.17(P〈0.05),术后3个月分别为0.30±0.14、0.22±0.13(P〈0.05);术后1、3个月A组的最佳矫正logMAR远视力≥0.5的眼数分别占40.0%、76.7%,B组分别占81.3%、87.5%,A、B两组之间差异具有统计学意义(P〈0.05)。A、B、C三组术前黄斑FRT分别为(234.03±11.12)μm、(231.47±16.61)μm、(233.23±15.31)μm(P〉0.05);术后1周,A、B组FRT分别为(242.80±10.83)μm、(236.25±18.13)μm(P〉0.05);术后1个月分别为(252.43±15.60)μm、(238.94±17.58)μm(P〈0.05);术后3个月分别为(238.57±11.68)μm、(236.56±23.46)μm(P〉0.05)。结论糖尿病性白内障超声乳化联合折叠式人工晶体植入术后黄斑中心凹视网膜增厚,1个月�Objective To investigate the changes of foveal retinal thickness after phacoemulsifica-tion and foldable intraocular lens implantation in diabetic cataract using optical coherence tomography. Methods The prospective controlled method was applied in this study. Thirty patients of diabetic cataract (30 eyes, Group A, the observation group), 32 patients of simple age-related cataract (32 eyes, Group B, the control group), 30 healthy people (30 eyes, Group C, the normal group) were selected in First People ' Hospital of Changde from July 2015 to January 2016. Groups A and B were performed with cataract phaco-emulsification and foldable intraocular lens implantation. The routine examinations including uncorrected distance visual acuity, best corrected visual acuity, intraocular pressure, anterior segment, and fundus be- fore and after operation were performed. Optical coherence tomorgraphy (OCT) was used to measure foveal retinal thickness before surgery and one week, one month, three months after surgery. Results The preop- erative best corrected LogMAR visual acuity of Groups A, B, and C were 1.06 ± 0. 42, 0. 92 ± 0. 37, and - 0. 3 ± 0. 06, respectively. The postoperative best corrected LogMAR visual acuity of Groups A and B were 0. 35 ±0.06, and 0. 31 ±0. 34 in the first week after operation, respectively( P 〉0. 05), 0. 38 ±0. 19, and 0. 27 ±0. 17 in the first month after operation, respectively ( P 〈0. 05), and 0. 30±0. 14, and 0. 22 ±0. 13 in the third month after operation, respectively ( P 〈 0.05 ). The percentage of the postoperative best corrected LogMAR≥0. 5 were 40. 0 % and 76.7% in the first month and the third month after operation in group A, and 81.3 % and 87. 5% in group B ( P 〈0. 05). The preoperative foveal retinal thick- ness (FRT) of Groups A [ (234. 03 ± 11.12)μm], B [ (231.47 ± 16. 61 )μm], and C [ (233.23 ± 15.31 )μm] were statistically significant, respectively ( P 〉 0. 05,) ; The postoperative FRT of Groups A an
