自体移植物和异体移植物在膝关节前后交叉韧带重建术中的应用效果对比

摘　　要：目的探讨自体移植物和异体移植物在膝关节前后交叉韧带重建术中的应用效果,为治疗方案的选择提供参考依据。方法将本院2010年3月至2014年3月收治的膝关节前后交叉韧带损伤的150例患者随机分为自体移植组和异体移植组,每组各75例。比较两组患者手术情况、治疗前后疼痛情况及膝关节功能评分变化情况。结果自体移植组患者手术时间显著长于异体移植组(P<0.05),住院时间显著短于异体移植组(P<0.05)。术后1天和术后2天,异体移植组患者视觉模拟评分法(visual analogue scale,VAS)评分均显著低于自体移植组(P<0.05)。术后1年,两组患者Lysholm评分、Larson评分及国际膝关节文献委员会(International Knee Documentation Committee,IKDC)膝关节评估表主观评分较术前均显著增加(P<0.05);术前及术后1年,两组患者膝关节功能评分组间比较差异均无显著性(P>0.05)。结论自体移植物和异体移植物在前后交叉韧带重建术中均具有良好的应用效果和安全性,对患者膝关节功能的改善均具有积极意义,临床治疗中可根据患者意愿选择移植物。Objective To investigate the therapeutic effect of autograft and allograft in arthroscopic reconstruction of anterior and posterior cruciate ligament, and to provide reference for the selection of treatment plans. Method 150 cases of anterior and posterior cruciate ligament injuries of knee joint patients treated in our hospital from March 2010 to March 2014 were randomly divided into autografl group （75 cases） and allograft group （75 cases）. Compared the operation condition, pain function and knee scores before and after treatment between the two groups. Result The operation time of autograft group was significantly more than allograft group （P 〈 0.05）, the time of hospitalization was significantly shorter than allograft group （P 〉 0.05）. The first day, second day after operation, the VAS score of allograft group was significantly lower than autograft group （P 〈 0.05）. The scores of Lysholm, Larson and IKDC subjective knee evaluation had significantly increased in the two groups 1 year after treatment （P ~ 0.05）. Before and 1 year after treatment, there were no significant differences in knee function scores between the two groups （P 〉 0.05）. Conclusion The autograft and allograft under in arthroscopic reconstruction of anterior and posterior cruciate ligament have good efficacy and safety, and has positive significance to the improvement of their wills knee joint function of patients. We can chose the transplant according to their wills in clinical practice.
