摘　　要：目的调查经外周静脉置入中心静脉导管（PICC）置管出院患者对延续护理服务的需求，为探索PICC置管患者的延续护理服务模式制定相关措施提供依据。方法对2所三甲综合性医院共165例PICC置管出院患者进行问卷调查，调查内容包括PICC置管出院患者一般情况，对延续护理服务内容、方式、满意度及需求。结果PICC置管出院患者延续护理需求较高，各延续护理服务方式合计非常需要占39．77％，需要占39．09％，但满意度较低，不满意占46．34％，满意仅占34．4％；不同年龄患者对建立健康教育网络平台和家庭访视需求差异有统计学意义（P〈0．05），不同地区患者对举办健康知识讲座、创建健康教育网络平台和病友俱乐部的需求差异有统计学意义（P〈0．05），不同文化程度患者对延伸护理需求差异无统计学意义。结论医院应建立PICC延续护理服务团队，根据患者需求及疾病情况制定个体化的延续护理服务方案，从而达到满足患者需求、提高PICC置管出院患者自我护理目的。Objective To identify the needs for transitional care among patients discharged with peripherally inserted central catheter （PICC） so as to provide evidence for measures regarding the form of delivering transitional care for patients with PICC. Methods Totally 165 patients discharged with PICC from 2 Tertiary Grade-A hospitals were surveyed with self-designed questionnaire covering general condition, as well as con- tent, forms, satisfaction, and demand of transitional care services. Results Patients discharged with PICC ex- pressed a high demand for transitional care. Across different forms of transitional care services, a total of 39. 77% of the patients needed transitional care ＇ very much＇, and 39. 09% patients needed transitional care. However, 46. 34% of the patients were not satisfied with the service, while only 34.4% were satisfied. The pa- tients in different age groups had different demand for health education website and home visit （ P 〈 0. 05 ）. Pa- tients living in different regions had different demand regarding health talks, health education website, and peer groups （P 〈 0. 05）. The need for transitional care showed little difference across patients with different educa- tional background. Conclusion Hospitals should establish transitional care team for PICC, providing individu- alized service according to patients＇ needs and diseases to meet the demand of patients and improve self-care after discharge in patients with PICC.
