苏拉明预防椎板切除术后硬膜外粘连形成的有效性及安全性

摘　　要：目的:比较局部应用不同浓度苏拉明预防腰椎椎板切除手术后硬膜外瘢痕粘连的效果。方法:48只SD大鼠用于制作L1椎板切除术模型,并将其随机分成4组,检测用药前后后肢运动功能评分(BBB评分)的变化。术后4周分别行大体标本观察、羟脯氨酸(HPC)含量检测,组织病理学观察,成纤维细胞计数以及胶原光密度值分析。结果:用药前后各组BBB评分差异无统计学意义(P>0.05),250 mg/m L苏拉明组无明显粘连,150 mg/m L苏拉明组存在轻度粘连,50 mg/m L苏拉明组和对照组形成紧密的硬膜外瘢痕粘连。250 mg/m L苏拉明组硬膜外瘢痕中HPC含量、成纤维细胞计数以及胶原光密度值较对照组明显降低,且优于150 mg/m L苏拉明组(P<0.05)。结论:局部应用苏拉明可减少椎板切除术后瘢痕形成,有效预防硬膜外粘连。而且局部应用250 mg/m L的苏拉明预防粘连效果更佳。Objective To determine the effect of Suramin by local application in preventing postlaminectomy epidural adhesion in rats. Methods Fory-eight mice of SD were modeled by laminectomy at the lumbar 1 level and were randomly divided into 4 groups. The BBB scoring was detected before and after administering Suramin. The macroscopic evaluation, hydroxyproline content, the histological morphological analysis and collagen density and fibroblast density were monitored in the 4th week after operation. Results BBB scoring had no significant difference before and after administration. The effect of Suramin in preventing epidural adhesion was shown in dose-dependent manner. Conclusion Topical application of Suramin has a good effect in preventing postlaminectomy epidural adhesion.
