摘 要:目的评估剪切波弹性成像(SWE)对新辅助化疗(NAC)后的病理反应性的预测价值。方法纳入进行NAC的乳腺癌患者58例,使用SWE评估治疗前肿物的平均硬度和最大硬度。记录NAC后病理反应性,并作为诊断金标准。采用Logistic回归分析得出影响NAC后pCR的独立影响因素并进行预测性诊断试验。结果 SWE平均硬度是pCR的独立影响因素。诊断试验证实平均硬度的预测效能最佳(AUC=0.82),且与Ki-67预测效能相比有统计学差异(P=0.04)。结论平均硬度预测pCR结局的效能较好,SWE可为乳腺癌的个体化治疗提供重要依据。Objective The prediction of shear-wave elastography(SWE) in response to neoadjuvant chemothera- py (NAC) was evaluated in breast cancers. Methods 58 breast cancers patients receiving NAC were included. The pre-treatment means and maximum stiffness were assessed by SWE. Pathological responses were recorded which was considered as gold standard. Logistic regression analysis was performed to explicit the independent influencing factors for pathological complete response (pCR), diagnostic test was also performed. Results Mean stiffness was the independent influencing factor. Diagnostic test suggested the predictive value of mean stiffness was the best (AUC = 0.82), and the difference of diagnostic performance between Ki-67 and mean stiffness was significant (P= 0.04). Conclusions Our study suggested the performance of mean stiffness for predicting the pCR was favorable, SWE may facilitates personalizing the treatment of breast cancer.
