摘　　要：目的探讨经食管超声心动图（TEE）在经胸微创封堵合并主动脉瓣脱垂的高位室间隔缺损中的应用价值和方法。方法对34例合并主动脉瓣脱垂的高位室间隔缺损的先天性心脏病患儿，实施经胸微创封堵术。其中嵴下型7例，嵴内型7例，嵴上型14例，干下型6例。TEE进行前评估筛选、术中引导和术后检测。结果34例中，31例（91．1％）封堵成功，随访5个月～2年，目前无任何并发症出现。手术开展早期有3例封堵不成功，改为体外循环下室间隔缺损修补术。结论TEE在经胸微创封堵合并主动脉瓣脱垂的高位室间隔缺损术前评估及术中引导均具有重要意义。Objective To evaluate the value of transesophageaI echocardiography （TEE） in transthoracic minimally invasive device closure of ventricular septal defect with aortic valve prolapsed（VSD-AVP）. Methods A total of 34 cases of VSD with AVP were recruited as candidates to receive transthoracic minimally invasive device closure between 7 infracristal VSDs, 7 infracristal VSDs, 14 supracristal VSDs arid 6 subpulmonic VSDs. The evaluation of the availability of device closure, procedure guided and monitored were all by TEE. Results A total of 31 cases（91.1%） were successfully closed with a device. All the patients were followed up more than 5 months arranged with a standard protocol. No thrombogenesis, complete atrioventricular block （ CAVB）, associated valvular dysfunction or other complications were observed. Three cases were failed to be closed by device and changed to surgical repair. Conclusions TEE plays an important role in transthoracic minimally invasive device closure of VSD-AVP which had been proved by the good follow-up results.
