顺铂致兔急性肾损伤肾脏血流灌注的超声造影研究

摘　　要：目的探讨顺铂致兔急性肾损伤(AKI)肾脏血流灌注的变化规律。方法雄性新西兰大白兔40只,经耳缘静脉注射顺铂建立兔AKI模型,取样时间分别为6(T6)、12(T12)、24(T24)、36(T36)、48h(T48),据此随机分为五组,每组8只。分别于建模前后相应时间点进行肾脏超声造影检查,采用Q-lab软件分析肾皮质、髓质TIC曲线。取耳缘静脉血检测建模前、后BUN及SCr值,AKI诊断指标参照KDIGO标准。使用SPSS22.0进行数据分析,P<0.05为差异具有统计学意义。实验结束后,取兔肾组织,常规石蜡切片,HE染色光镜观察。结果 T24、T36及T48组实验兔均发生AKI。建模后T12、T24、T36及T48组皮质RT,T24、T36及T48组皮质PI,T36及T48组皮质WIS、AUC较建模前具有显著差异(P<0.05)。建模后髓质各造影参数较建模前均无显著差异(P>0.05)。结论超声造影可灵敏地反映顺铂所致AKI早期肾皮质血流灌注的改变,RT在血BUN及SCr未达到AKI诊断标准前已有明显改变,PI、WIS及AUC的变化则出现较晚。Objective To explore the changing rule of renal blood perfusion in rabbits with acute kidney injury （AKI） caused by cisplatin. Methods 40 male New Zealand rabbits were divided into 5 groups randomly, every group contains 8 rabbits, named as the group T6 ,T12 ,T24 ,T36 and T48 respectively according to the chronological detection of the SCr,BUN and CEUS of the rabbits,the rabbit AKI models were made through injecting cisplatin on the ear vein, CEUS and SCr, BUN were detected on each group before and after the injection o＇f cisplatin, the renal cortical and medullar perfusion time-intensity curve（TIC） were plotted using Q-lab system. Use SPSS22.0 to analyze the data, P〈 0.05 for the difference was statistically significant. The kidney tissue of each group was gotten, routinely paraffin-embedded sections, the variations of the tissue of each group were observed with HE dyeing under light microscope. Results Compared with the data pre-injection, the cortical RT of T12, T24, T36 and T48 group, PI of T24, T36 and T48 group, WIS and AUC of T36 and T48 group varied significantly. All of the medullar CEUS parameters had an insignificant changes in comparison with the data pre-injection. All the rabbits of T24 ,T36 and T48 groups got AKI successfully. Conclusions CEUS could evaluate the renal blood perfusion sensitively at the early stage of AKI caused by cisplatin, RT got an apparent change before the BUN and SCr reached the AKI standard, the change of PI,WIS and AUC happened later.
