椎间盘切吸术联合胶原酶溶解术治疗复杂性腰椎间盘突出症效果观察

摘　　要：目的探讨椎间盘切吸术联合胶原酶溶解术治疗复杂性腰椎间盘突出症的效果。方法选取复杂性腰椎间盘突出症患者60例,给予椎间盘切吸术联合胶原酶溶解术治疗,对治疗效果进行观察。结果 60例复杂性腰椎间盘突出症患者手术均顺利完成,按照Mcnab标准评价60例患者中显效41例(68.3%)、有效8例(13.3%)、无效11例(18.3%),总有效率81.7%;术中、术后未出现严重并发症;治疗前疼痛VAS评分为(6.8±1.5)分,术后7 d疼痛VAS评分为(2.5±1.1)分,差异具有统计学意义(t=17.9063,P<0.01);术后回访6个月,患者生活质量指数QOL评分为(2.2±1.4)分,较入院时的(4.4±1.6)分显著改善(t=8.0155,P<0.01)。结论椎间盘切吸术联合胶原酶溶解术治疗复杂性腰椎间盘突出症在很大程度上避免了开放手术,减轻了患者痛苦,安全系数高,疗效确切,能显著改善疼痛症状,提高生活质量。Objective To explore the effect of percutaneous lumbar diskectomy with collagen zymolysis in the treatment of complex lumbar intervertebral disc heraiation （CLIDH）. Methods Sixty cases of patients with CLIDH were selected and given percutaneous lumbar diskectomy with collagen zymolysis. Results 60 cases of CLIDH operation were all smoothly accomplished. According to the valuation standard of Mcnab, 41 （68.3%） cases were significantly effective, 8 （13.4%） cases were effective, 11 （18.3%） cases were ineffective and the total effective rate was 81.7%. There were no serious complications observed. Before the treatment, the VAS score was （6.8±1.5） and at the 7 th day, the VAS scone was（2.5± 1.1）. The difference have statistically significant （t=17.9063, P〈0.01）. In 6 months after surgery, the quality of life QOL index was （2.2±1.4） which was superior to the （4.4±1.6）before treatment （t=8.0155,P〈0.01）. Conclusion Percutaneous lumbar diskectomy with collagen zymolysis in the treatment of complex lumbar intervertebral disc heraiation greatly reduce the degree of difficulty on the patient＇s pain, the safety coefficient is high and with significantly curative effect.
