纵向带蒂包皮内板尿道成形术联合改良尿道吻合术在尿道下裂中的应用

摘　　要：目的 使用纵向带蒂包皮内板尿道成形术联合改良尿道吻合术对尿道下裂进行临床治疗效果分析。方法 选取2008年12月～2011年12月我院接受尿道下裂治疗的患儿23例作为研究对象,实施纵向带蒂包皮内板尿道成形术联合改良尿道吻合术;选择同期住院的尿道下裂其他术式作为对照组,其中Duckett手术16例,Duckett＋duplay术式18例,Snodgrass术式19例。比较三组不同术式治疗效果。结果 随访观察了1.5年,观察到发生尿瘘14例,占18.42%;吻合口狭窄13例,占17.11%;尿道憩室11例,占14.47%,阴茎体旋转12例,占15.79%。结论 纵向带蒂包皮内板尿道成形术联合改良尿道吻合术对尿道下裂有着很好的临床治疗效果。Objective To observe the longitudinal with pedicle preputial urethral plate.forming surgery combined with modified urethral anastomosis of hypospadias were clinical therapeutic effect analysis. Methods Randomly selected in December 2008 to December 2011 came to our hospital for treatment of hypospadias in children in 23 cases, the implementation of longitudinal with pedicle preputial urethral plate forming surgery combined with modified urethral anastomosis; inpatients of hypospadias operation as control group, including 16 cases with Duckett surgery, Duckett＋duplay procedure was performed in 18 cases, Snodgrass in 19 cases. Compare the effect of different operation treatment in three groups.Results Follow-up for 1.5 years, observed and 14 cases of urinary fistula （18.42%）; 13 cases of anastomotic Strictrre, accounted for 17.11%; Urethral diverticulum in 11 cases, accounting for 14.47%, the penis body rotating 12 cases, accounting for 15.79%.Conclusion A longitudinal strip prepuce inner urethral plate forming surgery combined with modified urethral anastomosis of hypospadias has a good clinical effect.
