局麻和全麻对患者垂体前叶激素的影响

摘　　要：目的探讨了局麻和全身麻醉对患者血清垂体前叶激素(PRL、TSH、ACTH)水平的影响。方法将所有的子宫癌患者患者按照不同的麻醉方式来分为分为A组,采用硬膜外麻醉以及B组,采用静脉复合全麻,每组患者30例。在进行买醉诱导之前、手术切皮的过程当中以及手术开始一个小时以后,分别抽取3 m L的静脉血液来及进行ACTH、TSH以及PRL的测定。结果检测结果表明,A组血清PRL、TSH、ACTH含量在切皮和术中1 h与术前比较有显著差异(P<0.05),B组血清PRL、TSH、ACTH含量差异无显著性(P<0.05),在切皮和术中1 h,B组血清PRL、TSH、ACTH含量比A组含量降低(P<0.05)。结论静吸复合麻醉能明显降低血清PRL、TSH和ACTH浓度。Objective To explor the effect of different type of anethsia （regional vs general） on serum PRL、TSH、 ACTH lev- els of clinical analysis. Methods serum PRL、TSH、 ACTH levels were measured with RIA determined for 3 times in 30 patiems oper- ated under regional anesthesia and 30 patients operated under general anethesia （for carcinoma uterus）. The levels were measured be- fore induction of anesthesia, at beginning of operation and lhr later. Results in patients under regional anesthesia. Both the PRL、 TSH、 ACTH levels increased significantly at the beginning of operation and lhr later though dropped remained significantly higher than the levels before induction （ P〈0. 05）. No significant charges of the levels were obsorved in patients under general anesthesia throughout the operation, and the levels as a whole signicant lower than the levels under regional anethesia （P〈0. 05）. Conclusion general anes- thesia （ combined intravenous and inhalation） could abolish the lower of serum PRL、 TSH、 ACTH levels during operation.
