摘　　要：目的了解医院感染鲍氏不动杆菌的分布及对抗菌药物的耐药性,为临床使用抗菌药物提供参考依据。方法对2013年1月-2015年12月临床分离1056株鲍氏不动杆菌的标本来源和科室分布作回顾性分析,采用MicroScan WalkAway 96全自动微生物分析仪进行细菌鉴定和药敏试验,对其耐药性的变化进行分析,耐药分析采用WHONET5.6软件。结果 1 056株鲍氏不动杆菌主要来自于痰液标本,科室分布以呼吸内科的检出率最高,达27.0%,其次是ICU占20.0%和神经外科占16.0%。临床分离的鲍氏不动杆菌对多种抗菌药物具有较高的耐药性,对碳青霉烯类药物的耐药性达85.0%,明显高于全国平均水平。结论医院临床分离的鲍氏不动杆菌主要来源以呼吸道标本为主,对大多数的抗菌药物均有较高的耐药性,甚至出现泛耐药及全耐药现象,应引起特别关注。OBJECTIVE To investigate the distribution and drug resistance of Acinetobacter baumannii strains cau‐sing nosocomial infections so as to provide guidance for clinical use of antibiotics .METHODS The specimens sources and distribution of departments where 1056 strains of A .baumannii were isolated from Jan 2013 to Dec 2015 were retrospectively analyzed .The strains were identified by using MicroScan WalkAway 96 automatic mi‐croorganism analyzer ,the drug susceptibility testing was conducted to analyze the change of drug resistance of the strains ,and the drug resistance was analyzed with the use of WHONET5 .6 software .RESULTS Of the 1 056 strains of A .baumannii that were enrolled in the study ,the majority were isolated from sputum specimens ;the detection rate was the highest in the respiratory medicine department （27 .0% ） ,followed by the ICU （20 .0% ） and the neurosurgery department （16 .0% ） .The clinical A .baumannii isolates were highly resistant to multiple antibiotics ,the drug resistance rate to carbapenems reached up to 85 .0% ,significantly higher than the average level in China .CONCLUSION The majority of the clinical A .baumannii isolates are isolated from the respiratory tract specimens and are highly resistant to most of the antibiotics;the pandrug‐resistant and full drug‐resistant strains have even emerged ,which should be attach great importance to .
