白细胞介素-17在脓毒症患者中表达及其临床意义研究

摘　　要：目的观察白细胞介素-17(IL-17)在脓毒症患者中的表达,探讨其与疾病严重程度的相关性,为临床诊治提供参考依据。方法收集2015年1月-12月住院72例脓毒症患者,根据感染严重程度分为脓毒症组(20例)、严重脓毒症组(24例)及脓毒症休克组(28例),患者入院24小时及治疗后7天分别应用ELISA检测白介素-17、降钙素原及白三烯B4水平,并与健康对照组比较。结果健康对照组、脓毒症组、严重脓毒症组及脓毒症休克组患者血IL-17检测结果分别是:21.58±4.89,23.95±7.10,43.84±13.28,75.31±23.04,四组差异有统计学意义(t=87.8687,P<0.05);脓毒症组、严重脓毒症组及脓毒症休克组血IL-17治疗前、后的水平分别是:(38.64±11.65)VS(21.25±5.64),(t=4.2667,P<0.05);(44.65±12.89)VS(26.45±6.87),(t=3.5204,P<0.05);(76.32±22.53)VS(34.54±13.46),(t=2.8018,P<0.05);差异均有统计学意义。结论 IL-17与脓毒症的严重程度呈正相关,是预测脓毒症患者死亡率的重要因子。OBJECTIVE To observe the expression of interleukin‐17 （IL‐17） in patients with sepsis and explore the correlation between the IL‐17 and severity of disease so as to provide guidance for clinical diagnosis and treatment . METHODS A total of 72 patients with sepsis who were hospitalized from Jan 2015 to Dec 2015 were enrolled in the study and divided into the sepsis group （20 cases） ,the severe sepsis group （24 cases） ,and the septic shock group （28 cases） according to the severity of the infection .The levels of IL‐17 ,procalcitonin ,and leukotriene B4 were determined by using ELISA after the admission for 24 hours and after the treatment for 7 days and were compared with those of the healthy control group .RESULTS The serum IL‐17 level was 21 .58 ± 4 .89 in the healthy control group ,23 .95 ± 7 .10 in the sepsis group ,43 .84 ± 13 .28 in the severe sepsis group ,75 .31 ± 23 .04 in the septic shock group ,and there was significant difference among the four groups （t=87 .8687 ,P〈0 .05） .The serum IL‐17 level of the sepsis group was （38 .64 ± 11 .65）before the treatment ,（21 .25 ± 5 .64） after the treatment（t=4 . 2667 ,P〈0 .05）;the serum IL‐17 level of the severe sepsis group was （44 .65 ± 12 .89） before the treatment ,（26 . 45 ± 6 .87） after the treatment（t=3 .5204 ,P〈0 .05）;the serum IL‐17 level of the septic shock group was （76 .32 ± 22 .53） before the treatment ,（34 .54 ± 13 .46） after the treatment （t=2 .8018 ,P〈0 .01） ,and there was signifi‐cant difference .CONCLUSION The IL‐17 is positively correlated with the severity of sepsis ,and it is an important factor for the prediction of mortality rate of the sepsis patients .
