慢性乙型肝炎患者血液透析后血清β2-微球蛋白、纤维化指标与血小板参数的变化观察

摘　　要：目的观察慢性乙型肝炎患者血液透析后血清β2-微球蛋白、纤维化指标与血小板参数的变化情况,为该类患者的诊断与治疗提供参考依据。方法选取2014年1月-2016月2月医院收治33例慢性乙型肝炎患者血液透析后为A组,同时期33例血液透析无乙型肝炎者为B组,33名健康同龄者为C组,将3组受试者血清β2-微球蛋白、纤维化指标与血小板参数进行比较,并比较A组中轻度、中度及重度慢性乙型肝炎患者的上述血液指标。结果 A组血清β2-微球蛋白水平为(31.23±3.41)ug/ml,明显高于B组(21.25±2.63)ug/ml及C组(1.15±0.20)ug/ml,血清纤维化指标与血小板参数也均高于B组及C组,B组上述指标则高于C组,A组中重度乙肝患者的检测结果高于轻度及中度乙肝患者,中度乙肝患者则高于轻度乙肝患者,差异均有统计学意义(P<0.05)。结论血液透析合并慢性乙型肝炎患者血清β2-微球蛋白、纤维化指标与血小板参数均呈现高表达状态,且肝炎严重程度对其影响较大,因此,应注意对血液透析合并慢性乙型肝炎患者上述指标的监测与改善。OBJECTIVE To observe the changes of serumβ2‐microglobulin ,fibrosis indexes and platelet parameters of patients with chronic hepatitis B after hemodialysis so as to provide guidance for diagnosis and treatment of such group of patients .METHODS A total of 33 patients with chronic hepatitis B who underwent the hemodialysis from Jan 2014 to Feb 2016 were assigned as the group A ,meanwhile ,33 patients without hepatitis B who received the hemodialysis were set as the group B ,and 33 healthy people were assigned as the group C .The serum β2‐micro‐globulin ,fibrosis indexes and platelet parameters were compared among the three groups of participants;the above blood indexes were compared among the patients with mild ,moderate ,and severe chronic hepatitis B in the group A .RESULTS The level of serum β2‐microglobulin of the group A was （31 .23 ± 3 .41）ug/ml ,significantly higher than （21 .25 ± 2 .63）ug/ml of the group B and （1 .15 ± 0 .20）ug/ml of the group C .The serum fibrosis indexes and platelet parameters were also higher in the group A than in the group B and the group C ;the above indexes of the group B were higher than those of the group C .In the group A ,the detection results of the patients with severe hepatitis B were higher than those of the patients with moderate ,mild hepatitis B ,and there was significant differ‐ence （P〈0 .05） .CONCLUSION The hemodialysis patients complicated with chronic hepatitis B show high expres‐sion state of the serumβ2‐microglobulin ,fibrosis indexes and platelet parameters ,which were influenced by the se‐verity of hepatitis ,therefore ,it is necessary to attach great importance to the monitoring and improvement of the above indexes of the hemodialysis patients complicated with chronic hepatitis B .
