纤维支气管镜吸痰治疗对重症肺部感染患者呼吸状态与炎性应激的影响研究

摘　　要：目的研究纤维支气管镜吸痰治疗对重症肺部感染患者呼吸状态与炎性应激的影响情况,以了解纤维支气管镜吸痰治疗在此类患者中的应用价值。方法选取2014年6月-2016年3月医院治疗46例重症肺部感染患者为研究对象,将46例患者随机分为对照组和观察组,每组各23例,对照组进行常规重症肺部感染治疗,观察组则在常规治疗的基础上再进行纤维支气管镜吸痰治疗,将两组患者治疗前和治疗后12h、24h及48h的呼吸状态与炎性应激指标进行比较。结果治疗后12h、24h及48h观察组肺功能指标FEV1、MMF及FVC水平分别为(2.24±0.18)L、(1.08±0.14)L/s、(2.68±0.21)L、(2.60±0.20)L、(1.24±0.16)L/s、(3.15±0.24)L及(2.84±0.23)L、(1.55±0.19)L/s、(3.36±0.27)L,其均好于对照组,其他呼吸相关指标也均好于对照组,血清炎性应激指标均低于对照组,差异均有统计学意义(P<0.05)。结论纤维支气管镜吸痰治疗可有效地改善重症肺部感染患者的呼吸状态与炎性应激程度,因此在重症肺部感染患者中的应用价值较高。OBJECTIVE To observe the influence of fiberoptic bronchoscopy sputum suction on respiratory state and inflammatory stress of patients with severe pulmonary infection so as to understand the application value of the fi‐beroptic bronchoscopy sputum suction in treatment of such group of patients .METHODS A total of 46 patients with severe pulmonary infection who were treated in the hospital from Jun 2014 to Mar 2016 were recruited as the study objects and randomly divided into the control group and the observation group ,with 23 cases in each group . The control group was given the conventional therapy of severe pulmonary infection ,and the observation group was treated with additional fiberoptic bronchoscopy sputum suction based on the conventional therapy .The respir‐atory state and inflammatory stress indexes were observed and compared between the two groups of patients before the treatment and after the treatment for 12 ,24 ,and 48 hours .RESULTS The levels of lung function indexes FEV1 ,MMF ,and FVC of the observation group were respectively （2 .24 ± 0 .18）L ,（1 .08 ± 0 .14）L/s ,and （2 .68 ± 0 .21）L after the treatment for 12 hours ,（2 .60 ± 0 .20）L ,（1 .24 ± 0 .16）L/s ,and （3 .15 ± 0 .24）L after the treatment for 24 hours ,（2 .84 ± 0 .23）L ,（1 .55 ± 0 .19）L/s ,and （3 .36 ± 0 .27）L after the treatment for 48 hours ,better than those of the control group .Other respiratory indexes of the observation group were better than those of the control group;the levels of serum inflammatory stress indexes of the observation group were lower than those of the control group ,and there was significant difference （P〈 0 .05） .CONCLUSION The fiberoptic bronchoscopy sputum suction can effectively improve the respiratory state and inflammatory stress degree of the patients with se‐vere pulmonary infection ,thus ,it has high application value in the patients with severe pulmonary infection .
