脊髓损伤后行无菌间歇性导尿术防治尿路感染的随机对照研究

摘　　要：目的比较脊髓损伤后行间歇性导尿术与膀胱冲洗法防治尿路感染的疗效。方法将2010年10月-2015年9月医院收治的脊髓损伤后排尿功能患者90例,随机分为治疗组与对照组,对照组46例行膀胱冲洗法,治疗组44例采用无菌性间歇性导尿术;对比分析两组患者在治疗15天及30天两个时间点的尿路感染率、细菌培养及菌种差异。结果治疗30d后,无菌间歇性导尿组的尿路感染发生率明显下降,并显著低于膀胱冲洗组,差异均有统计学意义(χ2=21.518,P<0.001);外源性感染约占46%,与膀胱冲洗组比较,差异有统计学意义(χ2=4.963,P=0.026)。结论无菌间歇导尿对降低脊髓损伤后患者的尿路感染率更加有效,对比膀胱冲洗有更好的临床应用价值。OBJECTIVE To observe and compare the effects of intermittent catheterization and bladder irrigation on prevention of urinary tract infection in patients with spinal cord injury .METHODS A total of 90 patients with spi‐nal cord injury who were treated in the hospital from Oct 2010 to Sep 2015 were enrolled in the study and randomly divided into the treatment group and the control group .46 patients were assigned as the control group and treated with the bladder irrigation ,and 44 patients were assigned as the treatment group and given the sterile intermittent catheterization .The incidence of urinary tract infections ,bacterial culture result ,and distribution of species were observed and compared between the two groups of patients after the treatment for 15 and 30 days .RESULTS The incidence of urinary tract infections of the intermittent catheterization group was remarkably decreased and was significantly lower than that of the bladder irrigation group （χ2 =21 .518 ,P〈0 .001） .The patients with exoge‐nous infections accounted for about 46% ,as compared with the bladder irrigation group ,there was significant difference （χ2 =4 .963 ,P=0 .026） .CONCLUSION The sterile intermittent catheterization is more effective in re‐ducing the incidence of urinary tract infections in the patients with spinal cord injury and has higher clinical applica‐tion value than the bladder irrigation .
