摘 要:目的探讨六西格玛管理对手术人员外科手消毒的影响。方法运用六西格玛管理方法对某三甲医院手术人员外科手消毒操作是否规范及消毒效果进行定义与测量;对外科手消毒操作规范率偏低、消毒后手部ATP生物荧光监测合格率偏低进行原因调查,发现影响外科手消毒的主要原因为手部皮肤舒适度差、培训及检查监督不到位、洗手设备用物便利性不足等;针对外科手消毒操作的主要影响因素,对外科手消毒操作从制定培训计划、建立检查监督制度、实施理论操作培训及考核、对洗手设备及用物、操作流程图等方面进行改进。结果应用六西格玛管理后,手术人员外科手消毒的规范率由70.14%提高至93.17%;手术人员外科手消毒后1min ATP生物荧光监测RLU值及手术结束后RLU值与干预前比较,差异均有统计学意义(P<0.05);干预后手术人员手消毒后1min及手术结束后外科手消毒合格率与干预前比较,差异均有统计学意义(P<0.05)。结论六西格玛管理能有效提高外科手消毒规范率及消毒效果。OBJECTIVE To explore the effect of six sigma management on surgical hand disinfection of operating room staff .METHODS The six sigma management method was employed to define the standard hand disinfection procedures of the operating room staff of a three A hospital and to observe the disinfection effect ,the causes of the low rate of standard hand disinfection operations and low qualified rate of ATP bioluminescence monitoring of hand disinfection were investigated so as to find out the major influencing factors for the surgical hand disinfection ,in‐cluding poor comfort level of hand skin ,insufficient training and examination ,and lack of convenience of hand washing facilities .In allusion to the major influencing factors for the surgical hand disinfection ,the surgical hand disinfection procedures were improved by starting from the development of training plans ,establishment of inspec‐tion and supervision system ,implementation of theoretical operation training and assessment ,and hand washing facilities as well as operation flow chart .RESULTS The rate of standard surgical hand disinfection of the operating room staff was increased from 70 .14% to 93 .17% after the six sigma management was implemented .There was significant difference in the ATP bioluminescence monitoring RLU value 1 min after the hand disinfection or the RLU value at the end of surgery before and after the intervention(P〈0 .05) .There was significant difference in the qualified rate of surgical hand disinfection 1 min after the hand disinfection or at the end of surgery before and after the intervention (P〈0 .05) .CONCLUSION The six sigma management can effectively raise the rate of stand‐ard surgical hand disinfection and improve the disinfection effect .
