外周血Gal-3、NT-proBNP在慢性心力衰竭患者中表达意义的研究

摘　　要：目的探讨慢性心力衰竭(CHF)患者外周血半乳糖凝集素(Gal)-3、N末端B型利钠肽原(NT-proBNP)表达水平的变化。方法选择89例CHF患者作为研究对象,采用酶联免疫吸附法、电化学发光法分别检测入选患者外周血Gal-3、NT-proBNP表达水平,分析其在不同类型心力衰竭[左心射血分数保留的心力衰竭(HFPEF)、左心射血分数降低的心力衰竭(HFREF)]、心功能分级患者中的变化,并进行相关性分析。结果 HFPEF、HFREF患者Gal-3、NT-proBNP表达水平比较,差异均有统计学意义(P=0.000);心功能Ⅱ级、Ⅲ级、Ⅳ级患者Gal-3、NT-proBNP表达水平比较,差异均有统计学意义(P=0.000);Gal-3与NT-proBNP呈正相关(r=0.230,P=0.030),与LVEF呈负相关(r=-0.533,P=0.000);NT-proBNP与LVEF呈负相关(r=-0.372,P=0.000)。结论外周血Gal-3、NT-proBNP可作为评判CHF患者病情危重程度的重要指标。Objective To explore the expression and clinical significance of galectin（ Gal）-3 and N-terminal B-type brain natriuretic peptide（ NT-proBNP） in peripheral blood of patients with chronic heart failure（ CHF）. Methods Eightynine patients with CHF were enrolled in the study,moreover,25 healthy subjects were served as control group. The expression levels of Gal-3,NT-proBNP in peripheral blood were detected by enzyme-linked immunosorbent assay（ ELISA）,electrochemical luminescence method,respectively. The expression levels of them in different kinds of heart failure and different cardiac functional grades were observed and compared,including 41 cases of HFPEF and 48 cases of HFREF as well as 40 cases of cardiav function grade Ⅱ,25 cases of cardiac function grade Ⅲ,24 cases of cardiac function grade Ⅳ. Meanwhile the correlation between Gal-3,NT-proBNP and left ventricular ejection fraction（ LVEF） was analyzed. Results There were significant differences in the expression levels of Gal-3,NT-proBNP between patients with HFPEF and patients with HFREF（ P〈0. 01）,moreover,there were significant differences in the expression levels of Gal-3,NT-proBNP between the patients with HFPEF or the patients with HFREF and healthy subjects（ P〈0. 01）. There were significant differences in the expression levels of Gal-3,NT-proBNP among patients with cardiac functuon grade Ⅱ,Ⅲ,Ⅳ（ P〈0. 01）. Moreover Gal-3was positively correlated to NT-proBNP（ r = 0. 230,P〈0. 05）,however,Gal-3 was negatively correlated to LVEF（ r =- 0. 533,P〈0. 01）,besides,NT-proBNP was negatively correlated to LVEF（ r =- 0. 372,P〈0. 01）. Conclusion The expression levels of Gal-3 and NT-proBNP in peripheral blood can be regarded as important indexes for evaluating severity of illness in patients with CHF.
