摘 要:目的探讨血清CK-MB、ALT及BUN对新生儿窒息早期诊断的特异性或新生儿窒息严重程度的意义,为早期诊断和治疗提供理论依据。方法临床收集窒息新生儿80例,分为两组对比研究:轻度窒息组40例(轻度组)、重度窒息组40例(重度组);并选择40例健康新生儿(健康组)作为对照组,分别监测治疗前、治疗后血清CKMB、ALT及血BUN值的变化,进行统计学分析。结果治疗前轻度组、重度组分别与对照组比较,血清CK-MB、ALT均明显升高,差异有统计学意义(P<0.05);重度较轻度组比较,血清CK-MB、ALT及血BUN亦有明显升高,且差异有统计学意义(P<0.05);重度组治疗前后,血清CK-MB、ALT明显下降,相比差异有统计学意义(P<0.05);血BUN治疗前后有明显改变,差异有统计学意义(P<0.05)。轻度组治疗前后,血清CK-MB、ALT明显下降,相比差异有统计学意义(P<0.05);血BUN治疗前后没有明显改变,差异无统计学意义(P>0.05)。重度组治疗前CK-MB均与ALT、BUN值之间分布呈直线相关关系,(相关系数r=0.736,P<0.05;r=0.621,P<0.05)。轻度组治疗前CK-MB与ALT值之间分布呈直线相关关系,(相关系数r=0.890,P<0.05)。CK-MB与BUN值之间分布不成直线相关关系,(相关系数r=0.021,P>0.05)。结论血清CK-MB、ALT检测值的变化可以作为新生儿窒息的早期诊断数据依据,血BUN值在新生儿窒息早期无明显的变化,但在重度窒息中表达明显,可以用于临床中判断新生儿窒息严重程度的指标。Objective To investigate the significance of changes of myocardial-specific isoenzyme of creatine kinase ( CK-MB) , alanine aminotransferase ( ALT) and blood urea nitrogen ( BUN) levels in early diagnosis of neonatal asphyxia in order to provide theoretical basis for clinical diagnosis and treatment of neonatal asphyxia.Methods Eighty patients with neonatal asphyxia were divided into two groups: mild asphyxia group ( n =40 ) and severe asphyxia group ( n =40 ) , moreover, the other 40 healthy neonates were served as control group.The levels of CK-MB, ALT, BUN before and after treatment were detected and compared among the three groups.Results As compared with those in control group before treatment,the serum levels of CK-MB, ALT in mild asphyxia group and severe asphyxia group were significantly increased (P〈0.05).As compared with those in mild asphyxia group,the levels of CK-MB,ALT,BUN in severe asphyxia group were significantly increased ( P 〈0.05).Before treatment the serum levels of CK-MB,ALT in severe asphyxia group were significantly decreased ( P 〈0.05),moreover, there were significant differences in the levels of BUN before treatment and after treatment ( P 〈0.05).There were significant differences in the serum levels of CK-MB,ALT in mild asphyxia group before treatment and after treatment ( P 〈0.05) ,however, there were no significant differences in the levels of RUN before treatment and after treatment ( P 〉0.05).Before treatment there was a straight line correlation in the numerus distribution between CK-MB and ALT,BUN in severe asphyxia group ( r =0.736, P 〈0.05;r =0.621, P 〈0.05).Before treatment there was a straight line correlation in the numerus distribution between CK-MB and ALT in mild asphyxia group ( r =0.890, P 〈0.05),however, there was no straight line correlation between CK-MB and BUN ( r =0.021, P 〉0.05).Conclusion The detection of serum levels of CK-MB, ALT can be regarded as the indexes for early diagnosis of n
