介入治疗腹主动脉瘤合并髂内动脉瘤的临床有效性分析

摘　　要：目的探讨介入治疗腹主动脉瘤（AAA）合并髂内动脉瘤的临床效果。方法选取我院2015年1月-2016年1月收治的122例AAA合并髂内动脉瘤患者作为研究对象，依据治疗方案的不同分为对照组（60例）和观察组（62例）。对照组行传统开腹手术治疗，观察组行介入治疗。比较两组的并发症发生率、手术时间、住院时间、术中失血量、术后下地活动时间、ICU护理时间、正常饮食时间以及治疗后的白细胞、钾离子、丙氨酸氨基转移酶、血小板与尿氮素水平。结果观察组的并发症发生率为9．68％，显著低于对照组的23．33％，差异有统计学意义（P〈0．05）。观察组的手术时间、住院时间、术后下地活动时间、术后ICU护理时间以及正常饮食时间短于对照组，差异有统计学意义（P〈0．05）。观察组的术中失血量显著少于对照组，差异有统计学意义（P〈0．05）。两组治疗后的白细胞、钾离子、丙氨酸氨基转移酶、血小板与尿氮素水平比较，差异无统计学意义（P〉0．05）。结论临床给予AAA合并髂内动脉瘤患者介入治疗的效果确切，并发症发生率低，值得临床推广应用。Objective To explore the clinical effect of interventional therapy on treating abdominal aortic aneurysm （AAA） combining with internal iliac artery aneurysm.Methods 122 cases of AAA combining with internal iliac artery aneurysm in our hospital from January 2015 to January 2016 were selected and divided into the control group （60 cases） and the observation group （62 cases） according to the different therapeutic regimens.The control group was treated with traditional laparotomy,and the observation group was treated with interventional therapy.The incidence rate of complica tions,surgical time,hospital stay,intraoperative amount of bleeding,time to be off bed after surgery,nursing time in inten- sive care unit （ICU）,normal diet time,and the levels of white blood cell,potassiumion,glutamic-pymvie transaminase, blood platelet,and urea nitrogen were compared in the two groups.Results The incidence rate of complications in the observation group was 9.68%,which was lower than 23.33% in the control group,with significant difference （P〈0.05）.The surgical time,hospital stay,time to be off bed after surgery,nursing time in ICU,normal diet time in the observation group was shorter than that in the control group,with significant difference （P〈0.05）.The intraoperative amount of bleeding in the observation group was less than that in the control group,with significant difference （P〈0.05）.There was no signifi- cant difference in the level of white blood cell,potassiumion,glutamic-pyruvic transaminase,blood platelet,and urea ni- trogen between the two groups （P〉0.05）.Conclusion Application of interventional therapy for patients with AAA and in- ternal iliae artery aneurysm in clinic can obtain a definite effect and a low incidence rate of complications,it is worthy of clinical promotion and application.
