摘　　要：目的:描述团队协作服务的药学实践模式,分析该实践模式面临的机遇和挑战。方法:分别从服务对象、人员设置、硬件设施、服务流程和内容、结果等方面对团队协作服务中的药学实践进行描述,并查阅文献,与国内外药师参与门诊的业务模式进行比较。结果:已为近2000例患者提供药学服务,并实现收取药学服务费用。结论:团队协作服务是药师为患者提供药学服务的一种有效方法。Objective： To describe pharmaceutical services as a part of the inter- professional care in the menopausal clinic with related challenges and opportunities. Methods： We describe the pharmaceutical services from perspectives of patients, staff, hardware facilities, operation procedure and outcomes, and compared with models abroad by reviewing the literatures. Results： We have provided pharmaceutical services to approximately 2000 patients and were paid for the services. Conclusion： Inter-professional care is an effective approach for pharmacist to provide pharmaceutical services to the patients.
