摘 要:目的分析云南省德宏傣族景颇族自治州(德宏州)和红河哈尼族彝族自治州(红河州),艾滋病病毒(HIV)抗体阳性的配偶之间是否存在传播关系。方法在云南省红河州和德宏州,采集感染HIV的配偶双方的血液和对照样本,对所有样本进行病毒env和gag基因的分析,运用系统树和基因距离判断配偶间病毒的亲缘关系。结果共采集到了6对配偶的样本和45份对照样本,扩增成功了6对配偶和31份对照的样本。病毒亚型有01_AE、07_BC、08_BC和B亚型。经系统树分析,gag基因有4对配偶的序列,env基因有5对配偶的序列均在相同的簇内(可信度为100%和95%-100%),基因距离分别0.017(0.015-0.019)和0.031(0.012-0.059),与对照样本的差异有统计学意义(P〈0.01)。流行病学证据支持配偶间传播的问卷调查结果。另外1对配偶的基因分别归属于不同的亚型,基因间距为0.261,需要更多的证据来支持婚内传播关系。结论分子流行病学调查可以分析配偶间的HIV传播关系,在实际应用中该方法还需要更多的实验数据和流行病学资料来论证。Objective To determine HIV transmission methods within concordant spouses. Methods Blood samples were taken from HIV concordant spouses and other reference HIV infected persons in Honghe and Dehong prefectures of Yunnan province. HIV genes of env and gag were analyzed with phylogenetic trees and gene distances to determine their transmission relationships. Results Blood were taken from 6 pairs of HIV concordant couples and 45 reference persons. Subtypes of 6 pairs of spouses and 31 reference persons were determined with 01_AE, 07 _BC, 08_BC and B. The env and/or gag gene of 5 pairs of spouses were clustered together (credibility:95%-100%) and the genetic distances were between 0. 012 to 0. 059 that is statistically different from reference persons (P〈0.01). Molecular epidemiology of HIV transmission within 5 pairs of spouses was confirmed with investigation questionnaires of infection by positive spouse. The genes from one pair of spouses belonged to different subtypes and the genetic distance was 0.26. The data was not sufficient to support inter-spouse HIV transmission. Conclusion HIV transmission within heterosexual spouses can be confirmed by molecular epidemiology. More lab data and epidemiologic information is needed for the application of HIV molecular epidemiology research.
