摘 要:目的 探讨应用多排螺旋CT血管成像(multi-slice CT angiography,MSCTA)研究支气管动脉(bronchial artery,BA)的解剖特征及其临床价值.方法 120例咳血患者行MSCTA检查,男93例,女27例,年龄16~85岁,平均年龄52.4岁.对其数据进行容积再现(volume rendering,VR)、最大密度投影(maximum intensity projection,MIP)、多平面重组(multi-planar reformation,MPR)及曲面重建(curved planar reformation,CPR)等三维后处理,获得支气管动脉的起源、开口位置、分支类型等解剖信息.结果 对120例咳血患者的支气管动脉进行统计,发现起源正常支气管动脉271支(271/292,92.8%),异位起源支气管动脉21支(21/292,7.2%).分支类型R1L1型最常见占34.3%,其次较为常见的类型为R2L119.2%、R1L015.2%、R1L210.1%.右侧支气管动脉起源于右前壁(100/105,95.2%);左侧支气管动脉起源于左前壁(67/78,85.9%);共干支气管动脉起源于前壁(28/36,77.8%).结论 MSCTA可清晰显示支气管动脉,有效评价支气管动脉的三维影像解剖特征,对辅助支气管动脉栓塞介入手术插管具有肯定的临床价值,并能同时对肺部疾病进行评价.Objective To investigate the anatomic features and clinical value of bronchial artery with MSCTA. Methods 120 cases with hemoptysis were examined by MSCTA. Ninety-three cases were male, 27 were female. Their age ranged from 16 to 85 years old with the mean age of 524y. Post-processing techniques such as volume rendering (VR), maximum intensity projection (MIP), multi-planar reformation (MPR) and curved planar reformation (CPR) were used to analyze the MSCTA data to obtain the anatomical information of origin, open position and branch type with those bronchial arteries. Results The number of bronchial artery in 120 hemoptysis patients found that 271 branches (271/292,92.8%) were originated from the bronchial artery, 21 branches(21/292,7.2%) ectopic origin bronchial artery. The most common type of branch type R1LI was 34.3%, followed by R2L119.2%, R1L015.2%,R1L210.1%,. Right bronchial artery originated fi'om the right anterior wall in 95.2%(100/105) of patients; left bronchial artery originated from the left anterior wall was in 85.9%(67/78) of patients; stem bronchial arteries originated from the anterior wall in 77.8%(28/36) of patients.Conclusions MSCTA can clearly demonstrate the anatomical feature of BA, and provide guidance for intubation surgical treatment of hemoptysis. It has some role in clinical practice and can be used to evaluate lung diseases at the same time.
