中西医结合治疗慢性阻塞性肺疾病稳定期疗效评价

摘　　要：目的：评价中西医结合治疗稳定期慢性阻塞性肺疾病的疗效。方法：将60例患者分为治疗组与对照组各30例,治疗组在对照组单纯使用噻托溴铵粉吸入剂基础上加服中药汤剂,疗程3个月,观察两组疗效、证候与体征及肺功能的改善情况。结果：治疗组总有效率为96.67%（95%CI=82.75%～99.90%）,对照组为80.00%（95%CI=65.69%～94.31%）,两组综合疗效比较（u=1.760 2,P=0.078 4）,差异接近临界状态。与对照组比较,治疗组可降低中医证候咳嗽积分参数（t=-3.439 3,P〈0.05）,可能升高肺功能FEV1/FVC参数（t=1.942 2,P=0.059 0）。结论：采用中西医结合治疗方法可以有效缓解COPD稳定期症状与体征,改善患者肺功能,其收益为,OR=0.14（95%CI=0.02～1.23）,NNT=6（95%CI=3.04～238.10）。Objective： To evaluate the effect of therapy of integrated traditional Chinese and western medicine on stable phase of chronic obstructive pulmonary disease（ COPD）. Methods： 60 cases of patients were randomly divided into treatment group and control group,30 cases in each group. The control group was treated simple with Tiotropium Bromide Inhalation Powder. while the treatment group was treated plus decoction of traditional Chinese medicine on the basis of treatment of control group,3 months as a course of treatment. In both groups,the effects,syndromes and physical signs as well as improvements of pulmonary function were observed. Results： The total effective rate of treatment group was 96. 67%（ 95% CI = 82. 75% ~ 99. 90%）,while that of control group was80. 00%（ 95% CI = 65. 69% ~ 94. 31%）; compared the comprehensive effects of two groups（ u = 1. 760 2,P= 0. 0784）,the difference closed to the critical state. Compared with the control group,the treatment group could reduce the cough integration parameters of TCM syndromes（ t =- 3. 439 3,P〈0. 05）,and enhance FEV1/ FVC parameters of pulmonary function（ t = 1. 942 2,P = 0. 059 0）. Conclusion： The therapy of integrated traditional Chinese and western medicine can effectively alleviate the symptoms and physical signs of COPD stable phase,and mprove the pulmonary functions of patients. Its benefit is OR = 0. 14（ 95% CI = 0. 02 to 1. 23） and NNT6（ 95%CI = 3. 04 to 238. 10）.
