临床药师对1例急性重症肺炎患者的药学监护

摘　　要："以患者为中心"的临床药学实践提供一种药学监护模式,可确保临床用药安全、有效和经济。本文就临床药师通过对一例重症肺炎患者的抗感染药物治疗进行了监护,并及时跟踪了解患者病情变化,积极分析辅助检查结果,同时为患者抗感染治疗方案提供建议。因此临床药师可协同临床医师通过对重点案例的分析,优化治疗方案,实施药学监护,提升临床药师的药学监护能力和药学服务水平,促进临床合理用药。To provide a pharmaceutical care model for the practice of ＂patient-centered clinical＂ pharmacy so as to ensure safe,effective and economical clinical medication.In this paper,one case of severe pneumonia patients who had been treated with anti-infection was monitored by clinical pharmacist.Clinical pharmacists had timely follow- up to understand the patient＇ s condition changes,and actively analyzed the results of the auxiliary examination,and provided recommendations for the treatment of patients.It was noticeable that clinical pharmacists could cooperate with physicians to optimize treatment plan,calry out pharmaceutical care and promote clinical rational drug use.At the same time,clinical pharmacists could improve the pharmaceutical care ability and pharmaceutical service level by analyzing the key cases.
