胰岛素样生长因子治疗兔早期膝关节损伤的实验研究

摘　　要：目的:研究在兔膝关节腔内注射胰岛素样生长因子-1(IGF-1),治疗早期膝关节骨关节炎的效果。方法:将20只新西兰大白兔随机分为3组:A组(6只)为正常对照组;B组(7只)为膝关节骨关节炎治疗组;C组(7只)为膝关节骨关节炎对照组。其中,B组注射IGF-1工作液每次1 m L,每周1次,共注射4周。C组和A组关节腔内注射相同剂量的生理盐水,每周1次,共注射4周。4周后观察各组软骨及滑膜组织的大体形态、HE染色的病理切片,并进行Wakitani评分,检测关节液中白细胞介素-1(IL-1)的浓度。结果:大体肉眼观察可见,B组与C组比较,软骨修复情况较好。组织HE染色后光镜下观察可见,B组较C组软骨细胞存活增多,软骨基质结构完整,滑膜下层的纤维组织增生明显少。酶联免疫吸附法(ELISA)检测关节液中IL-1的浓度及Wakitani评分比较显示,各组间结果差异有统计学意义(P<0.05)。结论:关节腔内注射IGF-1对兔早期膝关节骨性关节炎差异有较好的治疗效果。Objective：To study the rabbit knee joint cavity injection of insulin - like growth factor 1（IGF - 1）,the effect of early treatment of knee osteoarthritis. Methods：Twenty New Zealand white rabbits were randomly divided into 3 groups：group A（6）as the normal control group;Group B（7）for knee osteoarthritis treatment group;group C（7）for knee osteoarthritis control group. Among them,group B：working liquid injection of IGF - 1,1 mL each time,once a week,a total of injection of 4 weeks. Intra - articular injection of Group A and Group C：the same dose of normal saline,once A week. A total of injection of 4 weeks. Observe each group after 4 weeks of cartilage and synovial tissue pathological gross morphology,HE staining,and Wa-kitani score,to detect joint fluid of interleukin - 1（IL - 1）concentration. Results：Generally visible to the naked eye observa-tion,Group B compared with Group C,cartilage repair is better. Visible organization after HE staining under observation,carti-lage cells survive Group B than in Group C,cartilage matrix structure integrity,synovial hyperplasia of fibrous tissue of the low-er significantly less. ELISA method to detect the concentration of the IL - 1 in joint fluid and Wakitani score,according to the comparison between each group results with statistical significance（P 〈 0. 05）. Conclusion：The articular cavity injection of IGF - 1 on early rabbit knee osteoarthritis has a better therapeutic effect.
