摘 要:目的探讨常规高频彩色超声与超声弹性成像联合应用对鉴别甲状腺良恶性结节的临床价值。方法选取2014年10月~2015年11月北京朝阳医院收治的55例行甲状腺手术患者,患者术前均行常规高频超声检查,再进行超声弹性成像检查,分析常规高频彩色超声和高频彩色超声联合弹性成像两种检查方法 ,并与术后病理比较,评价两种方法的诊断效能。结果 55例患者检出71个甲状腺结节,其中良性结节54个,恶性结节17个。与术后病理结果比较,常规超声和常规超声联合超声弹性成像的敏感度、特异度分别为52.94%(9/17)、83.33%(45/54)和88.24%(15/17)、98.15%(53/54),准确率、漏诊率、误诊率分别为76.06%(54/71)、47.06%(8/17)、16.67%(9/54)和95.77%(68/71)、11.76%(2/17)、1.85%(1/54),差异均有统计学意义(P〈0.05)。结论常规高频彩色超声与超声弹性成像联合应用可显著提高诊断的准确性。Objective To investigate the clinical value of conventional high-frequency ultrasonography combined with ultrasonic elastography in differential diagnosis of benign and malignant thyroid nodules. Methods Fifty-five patients undergoing thyroid surgery admitted to Beijing Chaoyang Hospital from October 2014 to November 2015 were selected.All patients were taken conventional high-frequency ultrasonography first before operation, followed by ultrasonic elastography. The conventional high-frequency ultrasonography and ultrasonic elastography were analyzed, and compared with postoperative pathological results, the diagnostic efficiency of the two methods was evaluated. Results There were71 thyroid nodules in 55 patients, among which, 54 thyroid nodules were benign, 17 thyroid nodules were malignant.Compared with postoperative pathological results, the sensitivity, specificity of conventional ultrasonography and conventional ultrasonography combined with ultrasonic elastography were 52.94%(9/17), 83.33%(45/54) and 88.24%(15/17),98.15%(53/54) respectively, the accuracy rate, rate of missed diagnosis, misdiagnosis rate were 76.06%(54/71),47.06%(8/17), 16.67%(9/54) and 95.77%(68/71), 11.76%(2/17), 1.85%(1/54), there were all statistically significant differences(P〈0.05). Conclusion Conventional high-frequency ultrasonography combined with ultrasonic elastography can significantly improve the accuracy of diagnosis.
