邱志楠治疗咳喘用药经验

摘　　要：慢性咳喘患者,病机以肺肾两虚为本,治疗应以扶正固本、温补肾阳为要,邱教授临证常用紫河车、淫羊藿,对于肾阳虚损的久咳顽喘患者,邱教授常加入熟附子;慢性咳喘病人病久入络,络脉不通,邱教授使用虫类药物止咳平喘,且认为虫类药物有抗过敏的作用;久咳顽喘患者,日久肺气上逆,邱教授常用酸枣仁、仙鹤草配伍使用,取其酸收之性;慢性咳喘病痰瘀互结证,邱教授常使用青天葵;对于久咳顽喘有瘀血之象的病人,邱教授常使用五味子、丹参药对。邱教授使用中医药治疗咳喘病具有个人的用药特色,在熟读古典医籍以及谙熟前人的相关经验基础上,借鉴现代中药药理学的研究结果,并验之于临床,遵古而不泥古。Due to long-term repeated attacks ,chronic obstructive pulmonary disease may consume lung qi ,damage the spleen and kidney,lead to phlegm turbidity accumulated inside the body,giving rise to stagnation and syndrome of qi deficiency and phlegm stagnation. The tip aspect focuses on the lung and its root focuses on the spleen and kidney. Yang deficiency of the spleen and kid- ney leads to disorder of raising lucid yang and descending turbidity, water fluid＇s failure in gasification;Then phlegm is accumula- ted for a long time to generate heat. Hot phlegm attacks upward, blocks the airway, giving rise to disorder of ascending and de- scending of the lung qi, so cough with asthma and phlegm appear. Phlegm is sticky and blocks qi activity,influences movement of qi and blood. When blood stasis appears ,or phlegm combined with blood stasis, chronic obstructive pulmonary disease. Two Matured Substances Decoction in combination with Lung-Draining Decoction can he used to activate the spleen and dry dampness, resolve phlegm and heat. For long-time deficiency of both lung and kidney,Ziheche （Placenta Hominis） may he added to warm the kidney and nourish the lung; For reversed qi due to the lung failing to descending qi, Wuweizi （Fructus Sehisandrae Chinensis ） may be added to astringe the lung, stop cough and calm asthma. For stagnation due to meridians invaded by disease, Danshen （Radix et Rhizoma Salviae Miltiorrhizae） may be added to activate blood circulation and dredge meridians. Yanshe（ Gekko Swinhonis Seu Japonicus） may be added to resolve phlegm and block meridians. The different drugs used together to stop cough and calm asthma, resolve and remove stagnation.
