中西医结合治疗肝细胞癌研究进展

摘　　要：肝细胞癌(简称肝癌)的病机是正虚邪实,正虚涉及肝、脾、肾,邪实则有气滞、血瘀、痰浊、癌毒等病理因素,临床常用BCLC分期指导该病治疗,主要分为BCLC 0期、A期、B期、C期、D期。根据肝癌的病因病机并结合肝癌的BCLC分期,可以遵循以下规律用药:初期治疗以疏肝健脾、补益气血、解毒抗癌祛邪为主,扶正为辅;中期治疗以健脾益气、疏肝和胃、解毒抗癌为主;晚期治疗以益气养阴、补益肾精、解毒抗癌为主。西药治疗过程中会出现不良反应,中西医结合能减轻患者临床症状,提高生存质量,延长生存期,减轻毒副作用,常用的中医外治法有:外敷疗法、穴位贴敷及穴位注射等。The pathogenesis of hepatocellular carcinoma （HCC） is deficiency of healthy qi which involves in the liver, spleen, kidney, and excess of pathogenic factors which have such pathological factors as qi stagnation, blood stasis, plalegm,cancer viruses, etc. In clinical practice, HCC is commonly treated under the guidance of BCLC staging which is mainly divided into BCLC 0, A, B, C, D stages, According to the etiology and pathogenesis of HCC and BCLC staging, the following drug-use principles can be followed：Early-stage treatment should focus on soothing the liver and invigorating the spleen, replenishing qi and blood, detoxification,anti-cancer and removing pathogenic factors;middle-stage treatment should focus on strengthening the spleen and replenishing qi, soothing the liver and regulating the stomach,detoxification,anti-cancer;late treatment should focus on supplementing qi and nourishing yin, tonifying the kidney essence,detoxification, anti-cancer. Adverse reactions will appear in the course of Western medicine treatment,the combination of Chinese traditional and Western medicine can relieve clinical symptoms of patients, improve the quality of life, prolong the survival time, reduce the toxic and side effects. The commonly-used TCM external treatment includes external application, point application and point injection.
