厦门地区2200例女性不孕患者AMH水平分析

摘　　要：目的探讨厦门地区女性不孕患者病因分布、年龄分布及其抗缪勒管激素(AMH)水平的改变。方法选择2014年1月至2016年1月来厦门生殖中心就诊的的2200例20-44岁不孕女性为研究对象,根据病因分为卵巢良性肿瘤组、多囊卵巢组、子宫肌瘤组、子宫内膜异位症组、盆腔炎组、输卵管不通组、其他病因(免疫、遗传)组以及因男方因素引起不孕的对照组。1190例女方因素引起不孕分3组:<25岁组、25-34岁组和>34岁组。AMH值用罗氏电化学发光法检测。结果女方因素不孕组中输卵管不通组所占比例最高21.0%,多囊卵巢组次之11.2%,其它组比例较少。不同年龄组25-34岁组所占比例最高52.9%。AMH值与年龄呈负相关,相关系数为0.32。除输卵管不通及其他病因组外,其余各组与对照组比较,差异均有统计学意义(P<0.01)。其中多囊卵巢组AMH值出现病理性增高,而子宫内膜异位症组、子宫肌瘤组和盆腔炎组AMH值均明显降低。结论病因和年龄是影响不孕女性AMH水平改变的重要因素。Objective：To examine the cause of the distribution,the age distribution and the level of anti-mulerian hormone. Methods：2200 cases of women with infertility visiting Xia Men reproductive center between Jan.2014 and Jan.2016 were chosen to divided into 8 terms according to the cause：the the group of ovarian benign tumor,the group of polycystic ovary,the group of uterine fibroids,the group of endometriosis,the group of pelvic inflammatory disease,the group of tubal nowhere,the group of the other causes,and the control group. 1190 cases of women with infertility（the cause of the women）were divided into three groups：younger than 25,between 25 and 34,older than 34.The level of anti-mulerian hormone was detected by the Roche method. Results：The group of tubal nowhere has a higher ratio in the 1190 cases,the level of the group of polycystic ovary was a little lower,but the other groups all has more lower ratio. The group of the age between 25 and 34 has a higher ratio about 52.9%.The level of anti-mulerian hormone had a negative correlation with the age,the correlation coefficient was 0.32.Without the groups of tubal nowhere and the other causes,the other groups all had a significant differences with the control group（P〈0.01）. The level of the group of polycystic ovary was higher,but the other groups′ level were lower about AMH. Conclusions：The causes and the age were all factors affecting the level of anti-mulerian hormone about women with infertility.
