补肾活血法在老年肱骨近端骨折围手术期的应用研究

摘　　要：目的：分析补肾活血法在老年肱骨近端骨折围手术期的疗效。方法：回顾2013年5月至2015年5月的79例老年肱骨近端骨折的患者,根据是否使用补肾活血汤分为治疗组和对照组。治疗组在常规对症治疗合并症同时,入院至术前1d和术后2~14d内,口服补肾活血汤1剂/d.对照组不服中药,其他与治疗组一致。结果：与对照组对比,补肾活血汤降低患者的疼痛程度（疼痛评分）,缩短肿胀天数,减少伤口浅表感染、术后发热、上静脉血栓例数,差异有统计学意义（P〈0.05）;而在切口愈合情况、压疮、肺部感染、尿路感染及原发老年病复合和加重方面,两组差异无统计学意义（P〉0.05）。结论：在老年肱骨近端骨折围手术期应用补肾活血法,可提高老年人耐受手术、创伤的应急储备能力,增强术后的抗感染能力,减轻患肢肿胀情况,促进患者早日康复。Objective：To analyze the efficacy of Bushenhuoxue decoction （BSHXD） on proximal humeral fractures （PHFs） of elderly patients in perioperative period. Methods： All 79 elderly patients with PHFs were selected in the retrospective study from May 2013 to May 2015. Based on drinking BSHXD, they were divided into control group and treatment group. The patients in the treatment group drank a dose of BSHXD one day in the times which were from the day of hos- pital admission to one day before operation and from the 2nd day to 14th day after operation, while they were generally treated the complications. And the control group was treated without BSHXD and other treatments were the same as the treatment group, Results：The pain VAS score, swollen times, rates of superficial infection, fever, and venous thrombus in the upper limb in BSHXD treated group were superior to the control group （P〈0. 05）. The healing times of the wound, the rate of bedsore, pulmonary infection, and urinary tract infection, and the recurrence and aggravating of pri- mary elderly diseases were not significantly different between the groups （P〉0. 05）. Conclusion： The application of BSHXD can improve the safety of elderly patients with PHFs in perioperative period and it is worth for application in the clinic.
