益气养阴活血通络方治疗2型糖尿病肾病临床研究

摘　　要：目的：观察益气养阴活血通络方治疗2型糖尿病肾病（diabetic nephropathy,DN）患者的临床疗效,分析其对患者血清胱抑素C、肿瘤坏死因子-α（tumor necrosis factor-α,TNF-α）及血尿酸的影响。方法：70例2型糖尿病肾病患者随机均分为观察组和对照组。对照组给予常规西医基础治疗,观察组则在此基础上给予益气养阴活血通络方。比较两组临床疗效,分析两组治疗前后血清胱抑素C、TNF-α、血尿酸及微量白蛋白的变化。结果：两组临床疗效相比,观察组治疗后显效率和有效率均显著高于对照组（χ^2=4.24、9.55,P〈0.05）;治疗后两组的空腹血糖、餐后2小时血糖、血尿氮和血肌酐较治疗前均有所改善,但组间比较,差异无统计学意义（P〉0.05）;治疗后,观察组各指标较治疗前均下降显著（P〈0.05）,且与对照组相比,差异有统计学意义（P〈0.05）,对照组仅血清胱抑素C较治疗前有明显下降（P〈0.05）。结论：益气养阴活血通络方治疗DN可有效提高疗效,降低血清胱抑素C、TNF-α和血尿酸水平,改善患者肾脏功能并缓解病情。Objective： To observe the effect of Yiqi Yangyin Huoxue Tongluo Decoction in treatment of type 2 diabetic nephropathy（ DN） in patients with clinical efficacy,analysis of the effect on serum cystatin C,tumor necrosis factor alpha（ TNF-α） and blood uric acid. Methods： 70 cases of type 2 diabetic nephropathy patients were randomly divided into observation group and control group. The control group was given routine treatment of Western medicine,the observation group was given on the basis of Yiqi Yangyin Huoxue Tongluo decoction. The clinical efficacy of two groups were compared,analysis of serum cystatin C,TNF-α,changes of blood uric acid and albumin of the two groups before and after treatment. Results： Compared the clinical efficacy of the two groups,the observation group after treatment,significant efficiency and effective rate were significantly higher than those in control group（ χ^2= 4. 24,9. 55,P〈0. 05）; after treatment,fasting blood glucose,2 hour postprandial blood glucose,blood urea nitrogen and serum creatinine of the two groups were improved than before treatment,but no difference between two groups（ P〈0. 05）. After treatment,the observation group each index before treatment were significantly decreased（ P〈0. 05）,and compared with the control group,the difference was statistically significant（ P〈0. 05）,the control group only serum cystatin C markedly decreased（ P〈0. 05）. Conclusion： Yiqi Yangyin Huoxue Tongluo Decoction in treating DN can effectively improve the curative effect,reduce the serum cystatin C,TNF-α and blood uric acid levels,improve renal function of patients and alleviate the condition,whichis worthy of clinical application.
