中西医结合治疗传染性单核细胞增多症临床研究

摘　　要：目的:观察自拟中药灌肠方联合阿昔洛韦治疗传染性单核细胞增多症的临床疗效。方法:将142例患者按随机数字表法分为对照组和治疗组,每组71例。对照组采用阿昔洛韦治疗,治疗组在对照组治疗基础上加用自拟中药灌肠方(金银花15 g,连翘15 g,白花蛇舌草20 g,大青叶10 g,板蓝根10 g,薄荷10 g,牛蒡子10 g,荆芥10 g,栀子10 g,黄芩10g,蒲公英15 g,生甘草5 g)治疗。治疗后观察两组患者的临床症状缓解状况、T细胞亚群、免疫球蛋白及心肌酶水平,并比较两组患者的临床疗效和不良反应。结果:对照组有效率为77.46%,治疗组有效率为90.14%,两组比较,差异有统计学意义(P<0.05);与对照组比较,治疗组临床症状缓解时间快,治疗后CD3+、CD8+水平降低,CD4+、CD4+/CD8+水平升高,治疗后Ig A、Ig G水平升高,治疗后天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、乳酸脱氢酶(lactic dehydrogenase,LDH)、肌酸激酶(creatine,CK)及同工酶(isoen zyme,CK-MB)水平降低,差异均有统计学意义(P<0.05)。结论:自拟中药灌肠方联合阿昔洛韦治疗传染性单核细胞增多症能改善患儿临床症状,降低心肌酶,恢复免疫功能。Objective： To explore the effect of self-made traditional Chinese medicine enema combined with acyclovir in the treatment of infective mononucleosis. Methods： The control group was given acyclovir treatment,the treatment group was treated with self-made traditional Chinese medicine enema. Composition： Jinyinhua（ Honeysuckle） 15 g,Lianqiao（ Fructus Forsythiae） 15 g,Baihuasheshecao（ Hedyotis Diffusa） 20 g,Daqingye（ Folium Isatidis） 10 g,banlangen（ Radix Isatidis） 10 g,Bohe（ Mint） 10 g,Niubangzi（ Fructus Arctii） 10 g,Jingjie（ Schizonepeta） 10 g,Zhizi（ Fructus Gardeniae） 10 g,Huangqin（ Scutellaria Baicalensis）10 g,Pugongying（ Dandelion） 15 g,Gancao（ Liquorice） 5 g. Clinical symptoms relief,T cell subsets,immunoglobulin and myocardial enzyme levels of two groups were observed after treatment,and the clinical efficacy and complications of the two groups were compared. Results： The effective rate of control group was 77. 46%,the treatment group was 90. 14%,the difference was statistically significant（ P〈0. 05）; the clinical symptoms relief of the treatment group was faster than that of the treatment group. After treatment,CD＋＋3,CD＋8levels decreased,CD＋4and CD＋4/ CD8 levels increased,the level of IgA and IgG raised,the levels of aspartate aminotransferase（ AST）,lactic dehydrogenase（ LDH）,Creatine Kinase（ CK）,and isoenzyme（ CK-MB） decreased,the differences were statistically significant（ P〈0. 05）. Conclusion： Self-made traditional Chinese medicine enema combined withacyclovir in treatment of infectious mononucleosis in children can improve the clinical symptoms,reduce myocardial enzyme and recovery of immune function.
