摘 要:目的:探讨低剂量对比剂在多层螺旋CT(multi-spiral CT,MSCT)肺动脉CT血管造影术(CT angiography,CTA)肺动脉栓塞检查中的影响。材料与方法:选取56例于我院接受肺动脉CTA检查的肺动脉栓塞确诊患者,根据随机的原则分为常规剂量组和低剂量组各28例,常规剂量组对比剂剂量为80ml,低剂量组对比剂剂量为25ml;测量并记录肺动脉主干、肺动脉左、右分支及左、右肺静脉增强后的CT值,比较两组图像质量。结果:两组肺动脉主干及左右分支强化后CT值比较差异没有统计学意义(P>0.05),两组肺静脉强化后CT值比较差异显著具有统计学意义(P<0.01),且低剂量组左右肺静脉增强后CT值显著低于常规剂量组;常规剂量组图像总体优良率为39.3%,低剂量组图像总体优良率为75.0%,两组比较差异具有统计学意义(P<0.05),低剂量组图像质量优于常规剂量组。结论:低剂量对比剂的应用可以显著提升图像质量,有利于肺动静脉的影像区分,在肺动脉CTA检查中可行性较高,值得临床推广应用。Purpose: To investigate the effect in the examination of low dose contrast agent in multi-spiral CT pulmo- nary artery CT angiography of pulmonary embolism. Materials and Methods: 56 cases of pulmonary embolism diagnosed in our hospital were selected for CTA examination, According to the principle of random, 28 cases were divided into conventional dose group and low dose group ; The dose of contrast medium in the conventional dose group was 80ml, and the close of contrast medium in the low dose group was 25ml; Measure and record the CT value of the pulmonary artery trunk, the left and right branches of the pulmonary artery and the right pulmonary vein, and compare the image quality of the two groups. Results: There was no significant difference in CT value between the two groups of pulmonary artery trunk and left and right branches ( P 〉 0.05 ) , There was significant difference in CT value between the two groups after pulmonary vein enhancement ( P 〈 0. 01 ), And in the low dose group, the CT value of the left and right pulmonary veins was significantly lower than that of the conventional dose group; The overall excellent and good rate of the conventional dose group was 39. 3%, the overall excellent rate of the low dose group was 75%, the difference was statistically significant ( P 〈 0. 05 ), The image quality of the low dose group was better than of the conventional dose group. Conclusion: The application of low dose contrast agent could significant- ly improve the image quality, it was beneficial to the pulmonary artery and vein image differentiation, and the feasibility was high of CTA examination in pulmonary artery , it was worthy of clinical application.
