亚临床甲状腺功能减退与2型糖尿病伴发视网膜病变的相关性分析

摘　　要：目的探讨亚临床甲状腺功能减退症(SCH)与2型糖尿病(T2DM)常见并发症的相关性。方法选取315例无甲状腺疾病史的T2DM患者,其中SCH组22例,甲状腺功能正常组293例,分析2组血管并发症情况及实验室指标,并进行Logistic回归分析。结果 T2DM患者中SCH的发生率为7.0%。T2DM合并SCH的患者周围神经病变、脑梗塞、心血管疾病、糖尿病肾病的患病率与甲状腺功能正常组无明显差别,但其视网膜病变的患病率比甲状腺功能正常组高差异具有统计学意义(P<0.05)。多因素Logistic回归分析显示病程、SCH是T2DM视网膜病变的独立危险因素。结论T2DM合并SCH患者视网膜病变患病率升高,应定期对T2DM患者进行甲状腺功能检查,及早干预SCH,延缓T2DM并发症的发生和发展。Objective To investigate the relationship between subclinical hypothyroidism（ SCH） and complications of type 2 diabetic mellitus（ T2DM）. Methods A total of 315 type 2 diabetic subjects without thyroid disease were enrolled,of which 22 subjects were with subclinical hypothyroidism and 293 subjects were normal. Vascular complications and laboratory indexes between the two groups were compared for Logistic retrospective analysis. Results The prevalence of SCH in T2 DM patients was7. 0%. The difference on the incidence of peripheral neuropathy,cerebral infarction,cardiovascular disease and diabetic nephropathy were not statistically significant between SCH group in T2 DM patient and euthyroid group. The prevalence of diabetic retinopathy was higher in SCH group than that in the euthyroid group,with the difference statistically significant（ P〈 0. 05）.Logistic regression analysis demonstrated that DM duration and SCH were independent risk factors for diabetic retinopathy. Conclusion The prevalence of diabetic retinopathy increased in SCH- T2 DM patients. Thyroid function should be regularly checked in type 2 diabetes patients to intervene SCH,prevent and delay the development of diabetic complications.
