摘 要:目的分析引起慢性化脓性中耳炎(CSOM)的铜绿假单胞菌(PA)超广谱β-内酰胺酶(ESBLs)及多重耐药情况。方法从CSOM患者中分离109株PA,分别使用全自动微生物分析仪、琼脂稀释法、ESBLs检测试剂盒进行细菌鉴定、药敏检测、ESBLs检测。结果多重耐药PA、产ESBLs菌株和产ESBLs多重耐药PA检出率分别为28.44%、47.71%、22.94%,其中骨疡型CSOM中检出率均高于单纯型CSOM(P<0.05)。ESBLs阳性PA对头孢他啶、阿米卡星、左氧氟沙星和复方新诺明耐药率明显高于ESBLs阴性PA(P<0.05或0.01)。结论 CSOM患者PA的耐药常见,以骨疡型CSOM最多见。Objective To analyze the extended spectrum β-lactamases and multiple drug resistance to Pseudomonas aeruginosa (Pa) isolated from chronic suppurative otitis media (CSOM). Methods Bacterial identification, drug susceptibility and ESBLs of 109 Pa isolates from CSOM were performed using automated microbiology analyzer, agar dilution, and ESBLs detection kit, respectively. Results The detection rates of multidrug resistant Pa, ESBLs- producing Pa and both were 28.44%, 47.71% and 22.94%, respectively; of wchich they were more common in carious type CSOM than in simple type CSOM (P〈0.05). Drug resistance to ceftazidime, amikacin , levofloxacin and trimethoprim-sulfamethoxazole in ESBLs-positive Pa strains was superior to that in ESBLs-negative Pa strains (P〈0.01 or 0.05). Conclusion Drug resistance of Pa is popular in CSOM, especially incarious type CSOM.
