摘　　要：目的观察穴位按摩、抱膝及徒手扩张宫颈口对足月妊娠单胎头位产妇第一产程活跃期停滞的影响。方法220例有第一产程活跃期停滞的足月妊娠单胎头位产妇随机分为干预组与对照组,干预组给予穴位按摩、抱膝及徒手扩张宫颈口,对照组仅予常规精神支持。观察两组阴道顺产率、宫口开全时间、产时和产后出血量、新生儿Apgar评分及母婴并发症情况。结果与对照组比较,干预组阴道顺产率较高,宫口开全时间缩短,新生儿Apgar评分较高,产时和产后出血量较少,尿潴留发生率较低,差异均有统计学意义(均P<0.05或0.01)。结论穴位按摩、抱膝及徒手扩张宫颈口可有效缩短第一产程活跃期停滞产妇的产程,降低母婴并发症。Objective To observe the effect of acupoint massage, embracing the knees, and unarmed cervical dilatation on active phase stagnation of the first labor stage in puerperae with full-term singleton pregnancy and cephalic presentation. Methods A total of 220 puerperae with full-term singleton pregnancy and cephalic presentation who developed the active phase stagnation in the first labor stage were randomly divided into intervention and control groups. Intervention group received acupoint massage, embracing the knees, and unarmed cervical dilatation, while control group accepted the conventional emotional support. The vaginal delivery rate, opening time of cervical orifice, intra- and post-partum bleeding, Apgar score and maternal and neonatal complications were compared between two groups. Results Compared with control group, vaginal delivery rate and Apgar score increased, but opening time of cervical orifice, intra- and post-partum bleeding, and postpartum urinary retention decreased in intervention group （P〈0.01 or 0.05）. Conclusion Acupoint massage, embracing the knees, and unarmed cervical dilatation can shorten the protracted active phase of the first labor stage in puerperae and reduce the incidence of maternal and neonatal complications.
