眼球后注射曲安奈德联合眼底激光治疗视网膜分支静脉阻塞继发黄斑囊样水肿的疗效观察

摘　　要：目的:探讨眼球后注射曲安奈德联合眼底激光治疗视网膜分支静脉阻塞继发黄斑囊样水肿的临床疗效。方法:选取2013年1月—2015年12月襄阳市中心医院收治的视网膜分支静脉阻塞继发黄斑囊样水肿患者100例作为研究对象,以随机数字表法分为观察组和对照组各50例。对照组患者给予眼底激光治疗,观察组患者给予眼球后注射曲安奈德联合眼底激光治疗。比较2组患者的治疗效果、视力水平、黄斑水肿厚度、中央视网膜厚度以及并发症发生情况。结果:观察组患者的总有效率为94%(47/50),显著高于对照组的78%(39/50),差异有统计学意义(P<0.05);治疗后,观察组患者的视力水平明显高于对照组,观察组患者的黄斑水肿厚度、中央视网膜厚度明显低于对照组,差异均有统计学意义(P<0.05);2组患者均未出现明显的并发症。结论:眼球后注射曲安奈德联合眼底激光治疗视网膜分支静脉阻塞继发黄斑囊样水肿,可有效改善视力和黄斑水肿厚度,安全性较高。OBJECTIVE： To probe into the clinical efficacy of retrobulbar injection of triamcinolone acetonide combined with laser photocoagulation in treatment of retinal vein obstruction secondary to cystoid macular edema.METHODS： 100 patients with macular edema secondary to branch retinal vein occlusion admitted into Xiangyang CentralHospital from Jan. 2013 to Dec. 2015 were selected to be divided into observation group and control group via the random number table,with 50 cases in each. the control group were treated with laser photocoagulation,while the observation group received retrobulbar injection of triamcinolone acetonide combined with laser photocoagulation. The treatment effects,visual acuity,macular edema thickness,central retinal thickness and incidence of complications were compared between two groups. RESULTS： The total effective rate of observation group was 94%（ 47 /50）,significantly higher than that of control group [78%（ 39 /50） ],with statistically significant difference（ P〈0. 05）. After treatment,the visual acuity of the patients in observation group was significantly higher than that in control group,and the macular edema thickness,central retinal thickness was significantly lower than those in control group,with statistically significant difference（ P〈0. 05）. There were no obvious complications in two groups. CONCLUSIONS： Retrobulbar injection of triamcinolone acetonide combined with laser photocoagulation in treatment of retinal vein obstruction secondary to cystoid macular edema can effectively improve the visual acuity and macular edema thickness with high safety.
