2013—2015年贵阳中医学院第二附属医院住院患者活血化瘀类中药注射剂应用分析

摘　　要：目的:了解贵阳中医学院第二附属医院(以下简称"我院")住院患者活血化瘀类中药注射剂使用情况。方法:统计2013—2015年我院使用量排序居前10位的活血化瘀类中药注射剂,随机抽取600份住院病历(每年每个品种各20份),分别对其用药适应证、用法与用量、联合用药等方面进行评价与分析。结果:600份病历中使用量排序居前10位的活血化瘀类中药注射剂共使用673例次,多用于心血管内科、神经内科、代谢内分泌科,其使用率分别为18.87%(127例次)、15.16%(102例次)、12.63%(85例次);2013—2015年我院急诊科活血化瘀类中药注射剂使用强度最高;妇科、骨科、肝胆外科等科室围术期的用药强度经点评整改后逐年得到控制;不合理用药主要包括适应证不合理、用法与用量不合理、溶剂选择不适宜及联合用药不合理等。结论:我院住院患者活血化瘀类中药注射剂的应用较广泛,但存在适应证、用法与用量、联合用药等不合理情况。应进一步加强点评干预等整改措施,使活血化瘀类中药注射剂的临床应用逐步规范化,确保药物治疗效果,减少药品不良反应。OBJECTIVE： To investigate the application status of promoting blood circulation Traditional Chinese Medicine（ TCM） injections in the Second Affiliated Hospital of Guiyang TCM College（ hereinafter referred to as ＂our hospital＂）. METHODS： The top 10 promoting blood circulation TCM injections ranked by consumption amount during2013-2015 were collected,600 inpatient medical orders were randomly extracted（ 20 orders every variety every year）to conduct the evaluation and analysis in terms of medication indications,usage and dosage and drug combination,etc.. RESULTS： Of the 600 medical orders,the top 10 promoting blood circulation TCM injections ranked by consumption amount was 673 cases,mainly for department of cardiovascular medicine,neurologyhas and metabolic endocrinology with the application rate of 18. 87%（ 127 cases）,15. 16%（ 102 cases） and 12. 63%（ 85 cases）. The use intensity of promoting blood circulation TCM injections in emergency department was the highest during 2013-2015. Perioperative application strength were controlled year by year in department of gynaecology,orthopaedics and hepatobiliary surgery. The irrational application were mainly irrational indications,usage and dosage,improper selection of solvent and drug combination. CONCLUSIONS： The application of promoting blood circulation TCM injections are relatively wide in our hospital,yet there are still some irrational phenomenon as indications,usage and dosage,drug combination. It is necessary to further strengthen the intervention and rectification to gradually standardize the clinical application of promoting blood circulation TCM injections,ensure the treatment effects and reduce adverse drug reactions.
