摘 要:目的探讨老年聋患者以远程的方式验配助听器后的助听效果。方法选取老年聋患者20人,随机分为两组:第一组10人,以远程的方式验配助听器;第二组10人,以面对面的方式验配助听器。分别采用言语测听及问卷评估的方式对配戴助听器前以及配戴助听器6N后的效果进行评价。言语测听材料选择普通话言语测听材料(mandarin speech test materials,MSTMs)的双音节辨别率测试,问卷选择国际助听器效果评定问卷(IOI-HA)。结果①两组患者助听下言语辨别率得分无显著差异(t=0.32,P=0.750〉0.05),②两组患者配戴助听器后问卷评估得分无显著差异(t=0.95,P=0.35〉0.05);③入组患者在安静环境下言语辨别能力配戴助听后显著优于未助听时的言语辨别能力(t=6.39,P=0.00〈0.05);④入组患者配戴助听器6周后问卷评估结果显示5%的患者对助听器效果满意,80%的患者认为助听器效果一般,15%的患者对助听器效果不满意。结论可以采用远程助听器验配技术对不方便来听力中心就诊的患者进行助听器验配及调试。助听器可有效提高患者在安静环境中的言语辨别能力。仍应通过双耳验配、助听辅助技术等方式提高患者在日常生活环境中的助听器使用满意度。Objective To evaluate the distance hearing aid fitting method used on the senior patients. Methods 20 senior hearing impaired patients were randomly divided into 2 groups. The first group used the distance hearing aid fitting method and the second group used the traditional face-to-face hearing aid fitting method. MSTMs speech audiometry and IOI-HA were used to evaluate the bearing aid effects of the two groups before fitting and 6 weeks after fitting.Results There were no significant differences between the speech recognition scores (SRS) of the two groups(t=0.32,P=0.750〉0.05).There were no significant differences between the questionnaire evaluation scores of the two groups(t=0.95,P=0.35〉0.05).The aided SRSs of the subjects in quiet environmental situation were significant higher than that of unaided ones(t=6.39,P=0.00〈0.05). The questionnaire evaluation scores showed that 5% of the subjects were satisfied with the hearing aid effects, 80% of the subjects gave a moderate evaluation, and 15% were unsatisfied.Conclusion The distance bearing aid fitting method could be used on the patients who were not convenient to receive the face-to-face hearing aid fitting. The speech discrimination ability in a quiet environment could be greatly improved by wearing hearing aids. The binaural hearing aids and hearing assistive technologies should also be used to improve the patients' hearing aid satisfaction in daily lives.
