2015年某院门诊不合理处方干预结果分析

摘　　要：目的提高合理用药水平。方法收集2015年1月至12月某院的门诊处方1 208张,查找存在的问题,对其适应证、用法用量和用药疗程等进行点评。结果用法用量不适宜处方302张(25.00%),临床诊断书写不全245张(20.28%),适应证不适宜处方87张(7.20%),疗程不合理处方65张(5.38%)。结论应不断规范医师处方行为,同时提升药师及时干预处方的能力,促使门诊处方用药安全、有效、经济、合理。Objective To improve the level of rational drug use. Methods A total of 1 208 outpatient prescriptions were collected from January to December in 2015, and the problems were identified. The indications, usage and dosage as well as treatment course were re- viewed. Results There were 302 prescriptions of inappropriate usage of dosage （25.00%）, 245 of incomplete writing of clinical diagno- sis （20. 28% ）, 87 of inappropriate indication （7.20%）, and 65 of irrational treatment course （5.38%）. Conclusion We should im- prove the professional ability of pharmacists and standardize the prescription behavior to guarantee the safe, effective, economical and ra- tional drug use of outpatient prescription.
