摘　　要：目的探讨宫颈长度对早产风险的预测价值。方法在我院门诊定期产前检查并住院分娩的孕妇401例,经会阴超声测量其宫颈长度并随访追踪妊娠结局,根据其妊娠结局分为足月组、先兆早产组及早产组,分析宫颈长度与妊娠结局的关系。结果足月组宫颈平均长度长于先兆早产组,先兆早产组长于早产组,三组差异均有统计学意义(P<0.05);不同宫颈长度对早产预测的灵敏度和阴性预测值随宫颈长度的增加而升高,特异度和阳性预测值随宫颈长度的增加而降低。足月组、先兆早产组及早产组宫颈长度≤30mm的阳性率分别为9.7%、32.8%、92.0%,差异具有统计学意义(P<0.05)。结论经会阴超声测量宫颈长度对预测早产具有一定的临床价值。对有先兆早产症状的孕妇在宫颈长度≤30mm时,早产发生率较高,可作为临床干预的警戒线。Objective To explore the value of measurement of cervical length by perineal Sonography in prediction of premature labor. Methods 401 cases of regular antenatal examination and institutional delivery in our out-patient clinic were selected, the cervical length through perineal sonography was tested and the pregnancy outcome was followed-up. The cases were divided into mature group, threatened premature labor group, and premature group according to the pregnancy outcome. The relationship between cervical length and pregnancy outcome were analyzed. Results The cervical length in mature group was longer than that in threatened premature labor group, and the cervical length in threatened premature labor group was longer than that in premature labor group. There are significant differences among the three groups（P 〈 0.05）. With the cervical length increasing, the sensibility and positive predictive value became higher. Besides, the specificity and negative predictive value became lower. As the cervical length was ≤ 30mm, the positive predictive values in mature group, threatened premature labor group and premature labor group were 9.7%,32.8%and 92.0% respectively. Conclusion There is a certainly significant value of cervical length through perineal sonography on premature labor. When cervical length of pregnant women with threatened preterm labor symptoms is less than or equal to 30 mm, it has a higher incidence of birth, which can be a warning line of clinical intervention.
