摘 要:目的研究非杓型高血压患者血压与基质金属蛋白酶9(MMP-9)及Chemerin蛋白水平昼夜变化的关系。方法收集符合纳入标准的原发性高血压患者118名为高血压组,正常血压的健康者28名为对照组,对所有入组者行24 h动态血压监测,根据检测结果将118例原发性高血压患者分为杓型组55例和非杓型组63例。均采用酶联免疫吸附法测定MMP-9及Chemerin蛋白水平,放免法测定肾素活性,化学发光法测量血管紧张素。比较高血压患者肾素-血管紧张素、MMP-9及Chemerin蛋白水平值并计算MMP-9、Chemerin蛋白水平和其他因素的相关性。结果高血压组的24 h平均收缩压水平较健康对照组显著升高(P<0.001),其中MMP-9、Chemerin蛋白以及肾素-血管紧张素的水平较对照组亦明显升高(P<0.01)。在高血压组的亚分组中,非杓型组较杓型组患者24 h平均收缩压显著增高(P<0.05),而夜间血压下降率显著减少,差异有统计学意义(P<0.05);非杓型组患者夜间肾素-血管紧张素、MMP-9和及Chemerin蛋白水平显著升高(P<0.01),且其昼夜差值差异也有统计学意义(P<0.01);非杓型高血压MMP-9及Chemerin蛋白水平与年龄、高血压病程、24 h平均收缩压、肾素、血管紧张素呈正相关(P<0.01)。结论MMP-9以及Chemerin蛋白与非杓型高血压昼夜变化水平相关。Objective To study the relationship between Non - dipper patients and the levels of matrix metalloproteinase - 9 ( MMP - 9 ) and Chemerin in day and night time. Methods 118 patients with primary dipper hypertensive were included in this study and 28 healthy individ- uals as control group. All the Participants were monitored by 24 h ambulatory blood pressure monitoring. According to the test results, the patients were divided into two groups, 55 dipper patients mad 63 non - dipper patients, respectively. We use ELISA t'o detect MMP - 9 and Chemerin pro- tein levels. The activity of renin were determined by radioimmunoassay kit. Angiotensin was detected using chemiluminescence. We compared the levels of renin- angiotensin, MMP- 9 and Chemerin. Moreover, we could observe correlations of MMP- 9, Chemerin protein and other factors. Results The average systolic blood pressure of 24 h in hypertension group increased significantly than that of the control group ( P 〈 0. 001 ), in which MMP-9, Chemerin protein and renin - angiotensin levels were also significantly higher than those of control group ( P 〈 0.01 ). In the sub - group of hypertensive group, the average systolic blood pressure of 24 h in the non - dipper group was significantly higher than that of dipper patients ( P 〈 0.05 ), while night - time blood pressure was significantly reduced, the difference was statistically significant ( P 〈 0.05 ). Renin -angiotensin, MMP-9 and Chemerin and protein levels of non- dipper patients at night were significantly increased ( P 〈 0.01 ), and the difference between day and night was also significant ( P 〈0.01 ). Non - dippers high blood pressure had positive correlation with age, duration of hypertension, 24 h mean systolic blood pressure, renin, angiotensin, MMP- 9 and Chemerin protein levels ( P 〈 0.01 ). Conclusion The non - dipper hypertension could associated with the MMP - 9 and Chemerin protein levels in day and night time.
