摘 要:目的探讨彩色多普勒超声对原发性高血压患者肾动脉血流动力学的评价作用及临床意义。方法从2012年8月到2014年8月就诊的原发性高血压肾病患者中100例作为观察组,再随机抽取100名健康人作为对照组。观察组患者以及对照组健康人均给予彩色多普勒超声检查,观察两组肾动脉血流动力学的变化规律,分析对比两组的检测结果。结果观察组检查者收缩压(173.43±35.71 mm Hg)以及舒张压(106.31±42.24 mm Hg)显著高于对照组(P<0.05);观察组检查者肾脏血流速度无论是最大血流速度还是最小血流速度均显著低于健康人的对照组观察组(P<0.05);观察组肾脏的长度(8.43±1.71 cm)以及肾脏实质厚度(5.23±1.22 cm)和肾窦厚度(6.24±0.85 cm)显著小于对照组(P<0.05);观察组肾脏的体积(70.44±11.22 cm^3)显著小于对照组(P<0.05);观察组患者尿β2-微球蛋白(0.91±1.71 mg/L),血尿素氮(11.23±1.22 mg/L)以及血肌酐(120.24±0.85 mg/L)均显著高于对照组(P<0.05)。结论利用彩色多普勒超声对原发性高血压患者肾动脉血流动力学进行检测,不仅能为肾损伤的早期诊断提供依据,还能对患者的预后进行评估,值得临床关注。Objective To explore role and clinical significance of the color doppler flow imaging (CDFI) in renal arterial blood flow dynam- ic in patients with essential hypertension. Methods From August 2012 to August 2014, 100 patients with primary hypertension nephropathy were se- lected as a observation group, then a random sample of 100 healthy people as control group. All patients were performed color doppler ultrasound ex- amination to observe the changing rule of the two groups of renal arterial blood flow mechanics. The results of two groups were compared and arm- lyzed. Results Inspectors systolic blood pressure ( 173.43 ± 35.71 mmHg) and diastolic pressure (42.24 ± 42.24 mmHg) in the observation group were significantly higher than those of control group ( P 〈 0.05). The inspectors renal blood flow velocity both maximum blood flow velocity ( 14.6 ±4.85) cm/s or minimal blood flow velocity (7.51 ±3.22 cm/s) in the observation group were significantly lower than those of the healthy control group ( P 〈 0.05 ). The length of the kidney (8.43 ± 1.71 cm), and renal parenchymal thickness (5.23 ± 1.22 cm) and the thickness of the renal sinus (6.24 ± 0.85 cm) in observation group were significantly less than those of control group ( P 〈 0.05 ). The kidney volume (70.44 ± 11.22 cm^3 ) in the observation group was significantly less than that of the control group ( P 〈0.05). Observation group of patients with urinary beta 2 - mieroglohulin as (0.91 ± 1.71 rag/L) , blood urea nitrogen for ( 11.23 ± 1.22 mg/L) and serum creatinine for ( 120.24 ±0.85 mg/L) were signifi- cantly higher than those of control group ( P 〈 0.05). Conclusion The utility of color doppler flow imaging (cdfi) in patients with primary hyper- tension renal artery blood flow mechanics test, can not only provide a basis for early diagnosis of the disease, but also can be used to evaluate the prognosis of patients with. Therefore, it is worthy of clinical attention.
