冠心病患者生活质量现状及影响因素分析

摘　　要：目的：探讨冠心病患者生活质量现状及其影响因素,为相关部门采取措施提高冠心病患者生活质量提供科学依据。方法：采用＂中国心血管病人生活质量评定问卷＂（CQQC）,对2014年12月~2015年12月我院心血管内科收治住院纳入标准的284例冠心病患者用笔试问卷和访谈形式相结合的方式评估冠心病患者生活质量以及影响生活质量的因素,并进行统计学分析。结果：经过笔试问卷调查发现,冠心病患者生活质量问卷得分为（78.62±28.31）分,处于中低等水平;通过访谈形式调查,冠心病患者生活质量影响因素分析,单因素分析,初筛出与CHD患者QOL有关联的因素分别为：患者年龄、性别、文化程度、婚姻状况、医疗费用来源、康复运动、治疗手段、病程;多因素Logistic回归分析结果采用多因素Logistic回归法对影响CHD患者QOL的因素进行分析,结果表明康复运动和病程是影响CHD患者QOL的危险因素。结论：冠心病患者生活质量处于中下水平,而且影响冠心病患者生活质量的因素是多方面的,包括患者年龄、性别、文化程度、婚姻状况、医疗费用来源、康复运动、治疗手段、病程等,而康复运动和病程是影响CHD患者QOL的重要危险因素。只有加强对冠心病患者生活质量影响因素的科学认识,才能采取切实可行的预防和干预措施,从而不断提高冠心病患者生活质量。Objective： To explore current status of quality of life and impact factors of patients with coronary heart disease and provide a scientific basis for the relevant departments to take measures to improve the quality of life of patients with coronary heart disease. Methods： A total of 284 patients with coronary heart disease admitted in our hospital dated from December 2014 to December 2015 in cardiovascular medicine department were treated with ＂ Chinese questionnaire of quality of life in patients with cardiovascular diseases＂（ CQQC）. A combination way of questionnaires and interviews was used to assess the quality of life of patients with coronary heart disease and impact factors of quality of life,and statistical analysis was conducted. Results： The scores of questionnaires of patients with coronary heart disease was（ 78. 62 ± 28. 31）,which was at the middle and low level. The interviews presented an analysis of impact factors for quality of life of patients with coronary heart disease. After the single factor analysis,the factors associated with QOL of CHD patients were age,gender,educational level,marital status,source of medical expenses,rehabilitation exercise,treatment and course of disease. Multivariate logistic regression analysis was used to analyze the influencing factors of QOL factors of CHD patients by using multivariate logistic regression method. The results showed that rehabilitation exercise and course of disease were two risk factors which influenced the QOL in patients with CHD. Conclusion： Many factors can lead the quality of life of patients with coronary heart disease in the middle and lower levels,and affect the quality of life of patients with coronary heart disease. These factors included patient age,gender,educational level,marital status,medical expenses,rehabilitation,treatment method and course of disease. Rehabilitation exercise and course of disease are important risk factors for QOL in CHD patients. Only by strengthening scientific knowledge of impact factors
