寻常型银屑病(白疕)血热证与TH17相关细胞因子病理变化的相关性分析

摘　　要：目的:探讨寻常型银屑病(白疕)血热证与TH17相关细胞因子病理变化的相关性,为治疗白疕血热证寻找靶细胞。方法:以白疕血热证患者为研究对象,在首诊时记录一般情况、临床表现(PASI评分、中医主要症状评分和HE病理),检测外周血血清TH17相关细胞因子。分析白疕血热证患者外周血中TH17相关因子的表达水平与临床表现(PASI评分、中医主要症状评分和HE病理切片中表皮厚度)的相关性。结果:记录58例白疕血热证患者,PASI评分平均(12.21±5.25)分;中医主要临床症评分平均(17.00±4.62)分;皮损组织HE病理中平均表皮厚度平均(153.2±30.4)μm;ELISA法检测血清中细胞因子IL-17a为(232.38±47.38)pg/m L、IL-1β为(23.71±9.73)pg/m L、INF-γ为(43.29±6.28)pg/m L、IL-4为(17.52±5.62)pg/m L。用Spearman检验进行分析,IL-17a与PASI评分、中医主要症状评分呈正相关(r=0.89,P<0.001;r=0.77,P<0.001),与表皮厚度不存在相关性(r=0.18,P=0.42);结论:白疕血热证与IL-17a(TH17分泌的主要细胞因子)病理变化呈正相关,IL-17a异常升高可能是白疕血热证重要的致病因素之一,而且TH17细胞及IL-17a可能是潜在的治疗白疕血热证的新靶点。Objective： To explore the correlation of the blood heat syndrome with psoriasis vulgaris and TH17 related cytokines, and search out the target cells of treating blood heat with psoriasis vulgaris. Methods： Recorded the patient＇s general condition, clinical manifestation （PASI score and TCM symptom scores and HE pathology）, and tested peripheral blood serum TH17 related cytokines. Then analyzed the correlation of blood heat syndrome patients in the expression level of TH17 related cytokines and clinical manifestations （PASI score and TCM symptom score and the thickness of epidermis）. Results： Recorded 58 eases of blood heat syndrome patients with the average PASI score： 12.21±5.25, average TMC clinical symptom score： 17.00±4.62, and tissue pathology HE average epidermis thickness： 153.2±30.4μm. Detected the serum cytokines IL-17a （232.38±47.38） pg/mL, 117113（23.71±9.73） pg/mL, the INF-gamma （43.29±6.28） pg/mL, IL-4（17.52±5.62） pg/mL.Correlation of the serum IL-17a and PASI score, the serum IL-17a and the TMC main symptom scures were positive （r=0.89, P〈 0.001; r=0.77, P〈0.001）, /he serum IL-17a and lhe thickness of epidermis is negative （r=0.18, P=0.42）. Conclusion： The correlation of the serum IL-17a and PASI score, the serum IL-17a and the TMC main symptom scores were positive. TH17 Related cytokines may play an important role in pathophysiology of psoriasis vulgaris, and it may be a potential Novel Drug Target of psoriasis vulgaris.
