COOK宫颈扩张球囊用于不同年龄段促宫颈成熟及引产的临床观察

摘　　要：目的观察不同年龄段产妇使用COOK宫颈扩张球囊促宫颈成熟效果以及引产情况。方法 选取2015年3月-2016年3月我院入院进行生产的96例孕妇,按孕妇年龄的不同分为观察1组（年龄23-〈35岁）、观察2组（≥35岁）,各48例。所有产妇生产过程中均使用COOK宫颈扩张球囊促宫颈成熟,以帮助产妇引产,观察不同年龄段产妇宫颈成熟情况、引产成功率、分娩情况,以及胎儿与母体情况。结果 观察1组、观察2组宫颈成熟率分别为95.8%、83.3%,差异有统计学意义（P〈0.05）;观察2组较观察1组第一产程时长、总产程长、产后出血量多、引产成功率低（P〈0.05）;观察2组新生儿窒息、胎儿窘迫、宫颈裂伤、阴道血肿发生率依次为16.7%、18.8%、29.2%、25.0%,高于观察1组的6.3%、4.2%、10.4%、10.4%（P〈0.05）。结论 临床采用COOK宫颈扩张球囊帮助产妇进行引产,其成功率较高,但对于不同年龄段产妇,≥35岁者较〈35岁产妇出血量多、引产成功率低、不良结局发生率高。Objective To observe the clinical effect of COOK urethral dilation balloon in the cervical ripening and labor induction for different age groups.Methods 96 pregnant women delivered in our hospital from March 2015 to March 2016 were selected and divided into observation Group 1 （23-〈35 years old）,observation Group 2 （≥35 years old） according to different age of pregnant women,48 cases in each group,COOK urethral dilation balloon was applied to improve the cervical ripening and labor induction.The cervical ripening condition,the successful rate of induced labor,the condition of delivery,and the condition of the fetus and the mother in different age groups were observed.Results The rate of cervical ripening in the observation Group 1 and the observation Group 2 was 95.8%,83.3%,and the difference was statistically significant （P〈O.O5）;compared with the observation Group 1,in the observation Group 2 the first dura- tion of labor and the total labor wan longer,postpartum hemorrhage volume was more,the success rate was low （P〈0.05）; incidence rate of neonatal asphyxia,fetal distress,cervical laceration,hematoma of vagina in the observation Group 2 was 16.7%,18.8%,29.2%,25.0%,higher than 6.3%,4.2%,10.4%,10.4% in the observation Group 1 （P〈0.05）.Conclusion The clinical use of Cook cervical dilation balloon to help women in labor,the success rate is high,but for the different age women aged over 35 years than under 35 years of maternal have more maternal bleeding volume,lower labor success rate,higher incidence of adverse outcomes.
