摘  要:目的通过调查近3年大连市友谊医院铜绿假单胞菌感染的分布情况及其耐药性变迁,为临床更加合理的使用抗生素提供依据。方法对大连市友谊医院从2012年7月—2015年6月住院患者中分离的1629株铜绿假单胞菌采用K-B纸片扩散法检测其对抗菌药物的敏感性,利用WHONET 5.6和SPSS 19.0软件进行统计分析。结果 1629株铜绿假单胞菌的主要标本来源为痰液和尿液,占91.22%;分布最广的科室为重症医学科,占31.18%,其次是综合病区,占21.91%;药敏结果显示头孢吡肟、氨曲南、头孢哌酮-舒巴坦、阿米卡星、环丙沙星、左旋氧氟沙星6种抗菌药物的耐药率均有下降,其中左旋氧氟沙星耐药率下降的相关性显著(P<0.05),具有统计意义;哌拉西林、氨曲南、左旋氧氟沙星3者的耐药率依然超过40%。结论铜绿假单胞菌是细菌感染中重要的致病菌,应加强对铜绿假单胞菌感染的管理,合理使用抗菌药物,减少耐药的发生。Objective To analysized the clinical distribution and the changes of drug resistance of Pseudomonas aeruginosa(PAE) so as to provide basis for the reasonable use of antibiotics. Methods The drug susceptibility testing was performed for PAE stains isolated from a variety of specimens from Dalian Friendship Hospital from July 2012 to June 2015, and the drug resistance results were statistically analyzed with the software of WHONET 5.6 and SPSS 19.0. Result The main sources ofPseudomonas aeruginosa are sputum and urine, accounting for 91.22%. The most widely distribution of PAE was the department of severe medicine, accounting for 31.18%, followed by the Comprehensive Department, accounting for 21.91%. Drug susceptibility results showed that resistance rates of 6 kinds of antibiotics decreased in the past tree years, including cefoperazone-sulbactam, cefepime, aztreonam, amikacin, ciprofloxacin, ofloxacin. Especially obvious in decrease to ofloxacin(P〈0.05). But the drug-resistance rates of piperacillin, aztreonam, ofloxacin were still more than 40%. Conclusion Pseudomonas aeruginosa is important pathogen of bacterial infection. The management of Pseudomonas aeruginosa infection should be strengthen to reduce the occurrence of drug resistance.
