螺旋空肠管在出血坏死性胰腺炎患者中的应用观察

摘　　要：目的探讨不同的鼻饲营养途径对出血坏死性胰腺炎患者疾病发展状况的影响。方法选择162例连续禁食、胃肠减压48h以上,且肠蠕动恢复、腹痛已缓解的出血坏死性胰腺炎患者按入院时间先后分成两组,对照组行鼻胃管鼻饲(70例),观察组鼻空肠管鼻饲(92例)。记录两组患者的并发症发生情况、淀粉酶恢复正常时间、白蛋白恢复正常时间、C反应蛋白恢复正常时间以及住院时间。结果观察组患者在出现感染、消化道出血、腹泻、腹腔脓肿、多脏器功能不全及死亡例数均显著少于对照组,差异有统计学意义(P<0.05)。观察组患者的淀粉酶恢复正常时间、白蛋白恢复正常时间、C反应蛋白恢复正常时间、住院时间均显著短于对照组,差异有统计学意义(P<0.05)。结论对出血坏死性胰腺炎患者护理中使用螺旋空肠管进行肠内营养优势明显,并发症显著减少,恢复时间短,护理难度低,值得临床推广。Objective To explore the effect on the development status of patients with hemorrhage necrotizing pancreatitis by different nasogastric nutrition channel. Method 162 bleeding and necrotic pancreatitis patients with continuous fasting, gastrointestinal decompression more than 48 h, and recovery of intestinal peristalsis, abdominal pain was selected and divided into two groups according to the admission time. The control group（70 cases）was used nasogastric tube , the observation group（92 cases）was used nasal jejunum nasogastric tube. The complications of the two groups were recorded, and the recovery time of amylase, the normal time of albumin recovery, ereaction protein and length of stay in hospital were recorded. Results The case of infection, gastrointestinal bleeding, diarrhea, ab- dominal abscess, multiple organ dysfunction and death cases in observation group were significantly less than that of the control group. There was significant difference （P 〈 0.05）. Patients return to normal time, propagated back to normal time,CPR returned to normal time, length of hospital stay in observation were significantly shorter than that of the control group, There was significant difference （P 〈 0.05）. Conclusion In the nursing care of patients with hemorrhagic necrotic pancreatitis, the advantages of spiral Jejunum tube for enteral nutrition were obvious. The com plications were significantly reduced, recovery time is short. The nursing difficulty is low. It is worthy of clinical promotion.
