胡思荣应用经方治疗初产妇产后抑郁经验

摘　　要：胡教授一般使用柴胡加龙骨牡蛎汤作为基本方治疗产后抑郁,伴有喜悲伤欲哭者,合用甘麦大枣汤。焦虑明显、而有热象者,合用百合地黄汤,或再加知母,成为百合知母汤、百合地黄汤、柴胡加龙骨牡蛎汤三方合方。若患者咽喉部症状明显,呈异物感者,使用柴胡加龙骨牡蛎汤合半夏厚朴汤。病人焦虑如狂、小腹胀满,表现为下焦蓄血证者,一般先用桃核承气汤,病情缓解后,再用柴胡加龙骨牡蛎汤合桂枝茯苓丸调理。除桃核承气汤等比较峻猛的方剂外,其他方药产妇可正常哺乳。有峻猛药物者,产妇服药2小时后亦可哺乳。胡思荣教授认为,产妇的情绪直接影响婴儿,所以中药不惟对产妇,对婴儿亦有帮助。从临床实践来看,的确婴儿食病母的乳汁后,没有发现不良反应,相反,情绪变得更稳定。Professor Hu commonly .uses Bupleu.rum Pluse Os Draconis and Concha Ostreae Decotion as the basic prescription for postpartum depression. For patients accompanied with sorrows and desire for crying, Licorice, Wheat and Jujube Decoction is combined. For server anxiety and heat manifestations, Lily Bulb and Rehmannia Decoction is combined or Zhimu （ Anemarrhe- na） can be added to form the combination of Lily Bulb and Anemarrhena Decoction, Lily Bulb and Rehmannia Decoction and Bupleurum Pluse Os Draconis and Concha Ostreae Decotion. For obvious symptom of the throat with foreign body sensation, Bup- leurum Pluse Os Draconis and Concha Ostreae Decotion combined with Pinellia and Officinat Magnolia Bark Decoction was ap-plied. For patients with mad anxiety and distension of the lower ＇abdomen and manifesting as syndrome of hlood retention in the lower energizer, Peach Kernel Qi-Guiding Decoction is generally firstly used. After remission of the disease condition, Bupleurum Pluse Os Draconis and Concha Ostreae Decoction combined with Cinnamon Twig and Poria Pill is used for regulation. Expect for violent formulas like Peach Kernel Qi-Guiding Decoction, other formula will not ＇,fffect the lactation. For drugs with strong effects,the puerperal can given lactation after 2 hours. Professor Hu thinks that the mood of the puerperal can directly affect the＇ infants.＂ Therefore, Chinese medicine is not simply effective for the puerperal but also for the infants. From clinical practice, the infants have no adverse reactions after sucking the milk instead, their mood becomes more stable.
