数据挖掘探讨2型糖尿病合并脂代谢紊乱证治规律

摘　　要：目的:探讨2型糖尿病合并脂代谢紊乱的中医证治规律。方法:基于"糖尿病中医结构化住院病历采集系统"中国中医科学院广安门医院内分泌科2006—2013年住院病历1 072例病历数据,运用贝叶斯网络分析方法进行数据挖掘,研究证与症状、方剂、理化指标之间的关系。结果:2型糖尿病合并脂代谢紊乱的证主要为气阴两虚血瘀证、气虚血瘀证,逐渐演变为阳虚血瘀证,湿痰、血瘀贯穿糖尿病合并脂代谢紊乱的始终;各期病情各个证的代表方剂为活血化瘀、益气养阴、清热化痰、温阳利水功效方剂,以丹参饮、玉女煎、生脉散、六味地黄汤、二陈汤、四物汤、五苓散、藿朴夏苓汤、五苓散等为主。结论:适宜的数据挖掘统计学方法是研究2型糖尿病合并脂代谢紊乱中医证治规律的有效手段。Objective：To explore the laws of syndrome and treatment for type two diabetes mellitus complicated with lipid metabolism disorder.Methods：The relationship between syndrome and symptoms,syndrome and prescriptions,syndrome and physical and chemical indexes were studied by analyzing the data of 1072 medical cases in the endocrinology department of Guang＇anmen Hospital,China Academy of Chinese Medical Sciences from 2006 to2013 based on ＂the information collection system of TCM structuralized inpatient cases related to diabetes mellitus＂,data mining was performed by using Bayesian network analysis method.Results：The patterns of type two diabetes mellitus complicated with lipid metabolism disorder were dual deficiency of Qi-Yin and blood stasis pattern,Qi deficiency and blood stasis pattern,which had gradually developed into Yang deficiency and blood stasis pattern,dampness-phlegm and blood stasis were throughout the whole period of type two diabetes mellitus complicated with lipid metabolism disorder;the representative prescriptions for different patterns at different periods were the ones of blood activating and eliminating stasis,invigorating Qi and nourishing Yin,heat-clearing and phlegm-eliminating,warming-Yang and dampness clearing;DanShen drink,YuNv decoction,ShengMai powder,LiuWei DiHuang Tang,ErChen Tang,SiWu Tang,WuLing powder,HuoPo XiaLing Tang,WuLing powder were the main prescriptions.Conclusion：Appropriate statistical method of data mining is the effective measure of studying the laws of syndrome and treatment for type two diabetes mellitus complicated with lipid metabolism disorder.
